All local areas are expected to submit narrative BCF plans but use of this template for doing so is optional. Although the template is optional, we encourage BCF planning leads to ensure that narrative plans cover all headings and topics from this narrative template. \nThese plans should complement the agreed spending plans and ambitions for BCF national metrics in your area’s BCF Planning Template (excel) . \nThere are no word limits for narrative plans, but you should expect your local narrative plans to be no longer than 15-20 pages in length. \nAlthough each Health and Wellbeing Board (HWB) will need to agree a separate excel planning template, a narrative plan covering morethan one HWB can be submitted, where this reflects local arrangements for integrated working. Each HWB covered by the plan will need to agree the narrative as well as their excel planning template. \nAn example answers and top tips document is available on the Better Care Exchange to assist with filling out this template. \nCover \nHealth and Wellbeing Board(s) \nHounslow Health & Wellbeing Board. \nBodies involved in preparing the plan (including NHS Trusts, social care provider representatives, VCS organisations, housing organisations, district councils) \nLondon Borough of Hounslow and NHS NW London-Hounslow Borough Team on behalf of the local Hounslow Health and Care system/BBP. The local BBP is made up of: \nHounslow council Hounslow Borough Team West London Trust \nWest Middlesex University Hospital Trust \nHounslow and Richmond Community Healthcare Trust Primary care \nVCSE partners and Hounslow Healthwatch. \nOver many years, LA and the borough leadership teams have been working together to commission services that drive integrated care pathways, cultures and ways of working. The shared objectives within the BCF are embedded and extended within the jointly agreed priorities set forthe BBP partnership. \nThe draft Hounslow Joint Health and Wellbeing Strategy 2022-2025 gives direction and sets priorities for all local partners who have a part to play in improving physical and mental health and wellbeing in the Borough, recognising the need to work together across organisations ensuring that equal emphasis is given to: \n• Community Involvement at all stages of service design \n• Cross organisational working, joint communication and integration with other services to reduce duplication and increase impact \n• Equal emphasis on mental as well as physical health and wellbeing \n• Reducing health inequalities \n• A focus on building resilience and independence so that people are enabled to help and make decisions for themselves \n• A focus on using the planning process to support health and wellbeing in the borough \n• Building social capital and a strong infrastructure to support the role carers and volunteers play in the wellbeing of Hounslow people, including those community organisations and groups of volunteers who provide health enhancing activities and deliver health benefits locally \n• A focus on prevention in all areas. \nIn support of integrated models of working Hounslow has a long history of effective joint commissioning across health, public health and social care. Governance is strong between the Borough Team and the Local Authority, with the Joint Commissioning Board promoting strategic discussions between organisations, ensuring we are aware of financial challenges and how we work together to meet the needsof a growing, complex population. The Better Care Fund Executive Board ensures we have strong governance around how we spend money together as well as ensuring the Borough Team and the LA understand the financial challenges they are facing and can analyse the impact the investments are having on non-elective admissions, delayed transfers of care and residential and nursing placements.
