THE FORM BEGINS ON PAGE 4.  \nDirections to Applicant-Public Service Recommendation Submission  \nImportant: The Public Service Recommendation must be dated after the chapter’s RUSH activity to be considered valid.  \nApplicants may submit the recommendation using one of the following options:  \nOption 1: Upload the Recommendation Yourself  \n• Request an official recommendation from the public service official.  \n• Ensure the recommendation meets all required criteria.  \n• Select “I will upload my recommendation” in the membership application.  \nOption 2: Request the Official to Upload the Recommendation  \n• Ask the public service official to upload the recommendation directly to your application.  \n• Select “I would like to request a recommendation in the membership application.  \n• Provide the official’s email address in the designated section.  \nNote: You may update the email address if needed.  \n• Once submitted, the official will receive an automated email with instructions to upload the recommendation.  \nNote: You will be able to view the uploaded recommendation before final submission.  \nDIRECTIONS & RECOMMENDATION REQUIREMENTS TO PUBLIC SERVICE ENTITY  \nPlease review the instructions for a list of required items to be included in this public service recommendation for the applicant.  \nThe Public Service Recommendation must be completed by the public service entity to verify the applicant’s service. It must be written by an official representative of the public service entity.  \nCollegiate Applicants  \n• Applicants must complete a minimum of twenty-five (25) hours of public service to the same entity fora period of three (3) months within the last 9 months prior to the RUSH.  \nAlumnae Applicants  \n• Applicants must complete 120 hours of service with the same entity within 24 months (2 years) prior the RUSH.  \nAPPENDIX 11J  \nPublic Service Recommendation Requirements  \nTo be considered valid, the Public Service Recommendation must include all required information listed below. Incomplete recommendations will result in the applicant’s packet being marked ineligible.  \n1. Author Eligibility  \n• The recommendation must be written by someone not related to the applicant.  \n• A relative is defined as someone connected by birth, marriage, or adoption.  \n2. Required Recommendation Details  \n• Applicant’s full name  \n• Name of the organization  \n• The recommendation must be typed, signed in pen and ink (digital signatures are not accepted), and dated.  \n• The date on the recommendation must include the month, day, and year (e.g. , April 30, 2023) and must be after the chapter’s official RUSH date.  \n3. Documenting Public Service  \n• On the recommendation, please complete the question in the designated box and provide specific, detailed information as requested. Your response will serve as your formal recommendation for the applicant.  \n• Formal Recommendation: Complete the question on page 5 under Applicant Recommendation and provide a response to the “What We Need to See” question.  \n• Documenting Public Service Hours: Complete the Public Service Hours Log on page 7. Each entry MUST include the month, year, and hours served.  \n4.
