There can be many levels of appeals, depending on the plan. If you have questions regarding a plan’s appeal levels or specific procedures, be sure to refer to that plan’s appeal guidelines. \nThe following sample letter can be helpful for you, the healthcare provider (HCP), and your office staff if it becomes necessary to appeal a coverage determination for a patient’s plan. The Appeal Letter covers details often required to process a coverage authorization appeal, including: \n• Claim specifics, such as the patient’s name, date of denial, claim amount, and group number \n• The patient’s medical history and course of treatment \n• The reason for denial: why the plan was reluctant to provide reimbursement \n• All necessary contact information \nThis guide and the following sample letter are presented for informational purposes only. They are not intended to provide reimbursement or legal advice. When in doubt, you are encouraged to contact third-party payers for specific information on their coverage policies. Teva recommends confirming the information that is required to include in an Appeal Letter with individual payers. \nINDICATIONS AND USAGE \nAJOVY® is indicated for: \n• The preventive treatment of migraine in adults, and \n• The preventive treatment of episodic migraine in pediatric patients who are 6 to 17 years of age and who weigh 45 kg or more. \nIMPORTANT SAFETY INFORMATION Contraindications: AJOVY is contraindicated inpatients with serious hypersensitivity tofremanezumab-vfrm or to any of the excipients. Reactions have included anaphylaxis and angioedema. \nHypersensitivity Reactions: Hypersensitivity reactions, including rash, pruritus, drug hypersensitivity, and urticaria were reported with AJOVY in clinical trials. Most reactions were mild to moderate, but some led to discontinuation or required corticosteroid treatment. Most reactions were reported from within hours to one month after administration. Cases of anaphylaxis and angioedema have been reported in the postmarketing setting. If a hypersensitivity reaction occurs, consider discontinuing AJOVY and institute appropriate therapy. \nConstipation with Serious Complications: \nConstipation with serious complications has been reported following the use of monoclonal antibody CGRP antagonists, including AJOVY, in the postmarketing setting. There were cases with monoclonal antibody CGRP antagonists that required hospitalization, including cases where surgery was necessary. \nMonitor patients treated with AJOVY for severe constipation and manage as clinically appropriate, including discontinuation. The concurrent use of medications that reduce gastrointestinal motility may increase the risk for more severe constipation and the potential for constipation-related complications. \nHypertension: Development of hypertension and worsening of pre-existing hypertension have been reported following the use of CGRP antagonists, including AJOVY, in the postmarketing setting. Monitor patients treated with AJOVY for new-onset hypertension or worsening of pre-existing hypertension, and consider whether discontinuation of AJOVY is warranted. \nRaynaud’s Phenomenon: Development of Raynaud’s phenomenon and recurrence or worsening of pre-existing Raynaud’s phenomenon have been reported in the postmarketing setting following the use of CGRP antagonists, including AJOVY. Many of the cases reported serious outcomes, including hospitalizations and disability, generally related to debilitating pain. \nAJOVY should be discontinued if signs or symptoms of Raynaud’s phenomenon develop. Patients with a history of Raynaud’s phenomenon should be monitored for, and informed a","cbCaiqmoNVCSy3BQ","https://ap.wps.com/l/cbCaiqmoNVCSy3BQ","pdf",257856,"English","# Appeal Letter Preparation Guide\n## When an Appeal Letter Is Needed\n## Appeals Process and Levels\n## Required Submission Materials\n## What to Include in the Appeal\n# Sample Appeal Letter Template\n## Payer and Patient Details\n## Treating Provider Statement\n## Denial Reason and Medical History\n## Contact and Attachment References\n# AJOVY Indications and Safety Information","[{\"question\":\"When should a healthcare provider submit an appeal letter?\",\"answer\":\"An appeal letter is needed when a patient’s health plan denies coverage following a medical-necessity determination.
