This agreement sets forth the terms under which ACPHD Care Partners as the subcontractor will provide services to the Food Bank in connection with 1) providing outreach and application assistance for improved access to the Supplemental Nutrition Assistance Program (SNAP), also known as the CalFresh Program in California, 2) promoting the Food Bank’s Emergency Food Helpline, and 3) sharing and securing data as detailed in the subcontract plan. \nLow-income seniors are particularly vulnerable to food insecurity, often lacking the resources necessary to access healthy and nutritious food on a daily basis. ACPHD’s Care Partners program, housed in the Community Health Services Division, reaches over 500 low-income seniors directly, and over 6,000 seniors indirectly through training and outreach to In-Home Health Services (IHSS) care providers. With this additional support and training, Care Partners will be able to address a much-needed gap in services to reduce hunger and food insecurity among our County’s most vulnerable seniors. \nAlameda County Community Food Bank | CalFresh Partnership Subcontractor Agreement \nAlameda County CalFresh Partnerships Subcontract Agreement \nThis Alameda County CalFresh Partnership Subcontractor Agreement (“Agreement”) entered into September 1, 2023 (“Effective Date”) between the Alameda County Community Food Bank Inc.
