This overview provides information on the appeal process through your insurance company, as well as information on filing for an independent review through the State of Connecticut External Review Program. \nWhy was my request denied? \nWhen you receive a denial notice, your insurance company is required to disclose to you the reason for the denial. These reasons might include: \n􀁸 Services are deemed not “medically necessary” \n􀁸 Services are no longer needed in that healthcare setting or level of care \n􀁸 The effectiveness of the health care services has not been proven \n􀁸 Services are considered \nexperimental/investigational for treatment of this condition \nIt is important to understand the reason why your request for services has been denied by your insurance company. This will enable you to work with your doctor to obtain medical documentation to support your need for these services. \nWhat are my rights as a consumer if I am denied services? \nIf you receive a denial based on the reasons above, you have the right to appeal this decision to your insurance company for another review(s) . \nIf you are unsuccessful, you have the additional right to have this decision reviewed by an Independent Review Organization which is not connected to your insurance company by applying for the State of Connecticut External Review Program. \nHow do I appeal this denial with my insurance company (Internal Appeal)? \nWhen your insurance company sends you notification that they have denied your preauthorization or claims request, they must also inform you of your right to appeal this decision. \nIf you disagree with the decision of the insurance company, you have 180 days to file a grievance (appeal) of this decision. Each denial letter from an insurance company will give you very specific information on how to file an appeal and where this request should be sent. \nIf you choose to file an appeal, it is important that you follow the appeals instructions printed in the denial letter and act within the designated timeframes. If you don’t file your appeal within these timeframes, you lose your rights to further review of the decision. \nWhat information is my insurance company required to provide upon request? \nTo assist you in your appeal, you are entitled to request from your insurance company “free of charge” reasonable access to, and copies of all documents, records and other information relevant to your request for services. Information on how to request this information is printed in your denial letter. \nWhat information is helpful to submit tomy insurance company when filing an appeal? \nIt is important that you send supporting documentation to your insurance company with your appeal. You should be aware that you have the right to ask your treating physician to provide information that would be helpful to your appeal. \nImportant information to submit with your appeal might include: \n􀁸 A letter of support from your treating physician indicating the medical reasons that the requested service should be approved \n􀁸 Treatment notes from your treating physician that provide information on the medical care provided to you \n􀁸 The results of any relevant tests or procedures related to the requested service \n􀁸 Your own personal narrative or the narrative of an authorized representative describing the need for the requested service \n􀁸 For experimental or investigational treatments, any current medical literature or studies documenting the medical efficacy of the requested services \nWhere may I receive free assistance in preparing my appeal? \nYou have the right to assistance in filing your app","cbCaiaTsxwsi5QeF","https://ap.wps.com/l/cbCaiaTsxwsi5QeF","pdf",459762,7,"English","# Introduction\n## Why requests may be denied\n## Consumer rights after a denial\n# Internal appeal with your insurer\n## Filing deadlines and instructions\n## Information the insurer must provide\n## Helpful documents to submit\n# Assistance and urgent care appeals\n## Where to get free help\n## Expedited handling for urgent appeals\n## Who reviews the appeal","[{\"question\":\"What reasons can a health insurance company give for denying services?\",\"answer\":\"Denials may cite lack of medical necessity, services no longer needed at that level of care, unproven effectiveness, or the service being experimental/investigational for the condition.\"},{\"question\":\"How long do I have to appeal a denial with my insurance company?\",\"answer\":\"You generally have 180 days to file a grievance (appeal) after receiving the denial notice.
