Any applicant (even if the person has a community spouse with low income) for any institutional Medicaid (including waiver) program with income of $2200* or more. To be eligible for institutional Medicaid, the maximum gross income allowed is three times the SSI Federal Benefit Rate (currently $733 x 3=$2199). Any deductions from the income, such as Medicare Part B, income tax withholding, or withholding for a Social Security overpayment, do not reduce the countable income for purposes of determining eligibility.
* There is a general $20 exclusion of income for SSI. JFS should apply that exemption to the special income level, but it is unclear if they will apply it.
What banks will accept qualified income trust accounts? See the handout attached. Do not assume either that a bank will or will not accept a QIT based solely on the handout. Ask the bank.
Where is the QIT template?
\u0013 HYPERLINK "http://medicaid.ohio.gov/Portals/0/Initiatives/DDR/QIT-template.pdf" \u0014http://medicaid.ohio.gov/Portals/0/Initiatives/DDR/QIT-template.pdf\u0015
Is there an entity that assists with QITS? Yes, Automated Health Systems, at 1-844-265-4722 or by email at \u0013 HYPERLINK "mailto:OhioQIT@automated-health.com" \u0014OhioQIT@automated-health.com\u0015
Will AHS assist with setting up QITs for future Medicaid applicants? According to their contract with ODM, yes. An applicant may request that the nursing facility (or AAA if a waiver application) file the application, and the applicant should immediately obtain the Medicaid pending number. The applicant should then call AHS with the Medicaid pending number and ask for assistance.
What will happen to current Medicaid recipients who have income in excess of the special income level above? The person should receive a letter referring the client to AHS. All recertifications for institutional Medicaid programs have been delayed until 2017, so current recipients should have until 2017 to have a QIT established, and AHS is contractually obligated to help the recipient. The sample letter can be reviewed here: \u0013 HYPERLINK "http://medicaid.ohio.gov/Portals/0/Initiatives/DDR/Letter-QIT-2016-04pdf.pdf" \u0014http://medicaid.ohio.gov/Portals/0/Initiatives/DDR/Letter-QIT-2016-04pdf.pdf\u0015
What are the logistics of setting up a QIT? First, the applicant needs to fill out and sign the QIT document. Second he/she should take the signed document to the bank and request the bank set up a second account, titled, “John Doe Qualified Income Trust.” Third, once the person is ready to file for Medicaid, an electronic transfer should be set up from Account A (the checking or savings account where John Doe’s income is deposited) to the QIT account. The amount transferred should be sufficient to leave the remaining deposit into account A at less than the special income level (with some allowance for future cost of living increases). E.g., John receives a Social Security check of $2000, and a pension of $600 per month. John should have an ongoing electronic withdrawal of $500 each month to the QIT. That would leave John’s countable income in Account A as $2100. It is not necessary that an entire source of income, or even just one source of income, be transferred each month. The amount not transferred from Account A needs to be less than the special income level.
What withdrawals are permissible from the QIT? Only the following items can be withdrawn from a QIT account. Any other withdrawal would be considered an improper transfer, incurring a potential restricted period of eligibility (from Dept. of Medicaid website):
“What can the money in the Trust be used for?
Money put in the QIT can be used to pay for the following expenses, but is not limited to this list:
» incurred medical expenses;
» monthly personal or maintenance needs allowance;
» bank fees associated with the maintenance of the Trust; and
» patient liability, if applicable.”
\u0013 HYPERLINK "http://medicaid.ohio.gov/Portals/0/Initiatives","cbCaimZWHt9gMX4h","https://ap.wps.com/l/cbCaimZWHt9gMX4h","docx",21014,"English","# 1634 FAQ’s
## Eligibility and income counting
## QIT template, banks, and AHS assistance
## QIT setup logistics
## Permissible withdrawals and penalties
## Other Medicaid rule changes and Community Medicaid notes","[{"question":"Who needs a qualified income trust for institutional Medicaid, and what income level applies?","answer":"Any applicant for institutional Medicaid (including waiver) with income of $2200 or more.
