DSS contracted with Xerox for this service .  \n􀂃 DSS staff were on site, full time, in the main call center two weeks following the mailing. DSS are also on-site daily supporting escalations . Call volume has been low.  \n􀂃 Staggered over the last week of January 2016, DSS mailed out over  \n640,000 1095-B forms to 2015 HUSKY Health members and authorized representatives . This includes HUSKY A, B, C and D.  \n• Only members of programs considered to provide “minimal essential coverage” or “MEC” received a form . Programs not considered to be MEC are those with limited benefits such as Family Planning or TB.  \n1095-B Points of Interest  \n􀂃 In addition to issuing 1095-B forms to households, DSS is required to electronically submit the same information to the Internal Revenue Service via 1094 file .  \n• This file will be sent at the end of March  \n• The 1095-B form is in effect a consumer’s copy of their details on that file .  \n􀂃 The 1095-B form does not have to be submitted with a consumer’s tax filing as the IRS is using the data from 1094 file . The form is for the consumer’s records.  \n􀂃 The 1095-B form should not be returned to DSS.  \nUpcoming Activities  \n􀂃 HUSKY B Band 3 (non-CHIP buy-in program) will get Individual Exemption Certificates as officially it is not a MEC, without certification from CMS.  \n• Since the program ended as a result of state legislation last year, DSS did not seek certification as MEC.  \n• Instead, DSS got permission from CMS to directly mail an Exemption Certification, i.e ., we are skipping the exemption application step.  \n• Mailing will arrive in an Access Health CT envelope and will include an explanatory cover letter.
