contrary,lep-rosy is still a world health problem of some magnitude.Accordingto the best estimates, this chronic, often excruciatingly painful, disfiguring and incapacitating disease may now afflict up to twelvemillion human beings. \nLeprosy is most prevalent in a wide global girdle which includesAfrica,India,China,South East Asia, the Philippines, and parts ofCentral and South America. In the Middle Ages,it was widespreadin Europe, and only within the past two decades has it disappearedfrom Britain and Scandinavia.Although long since gone from mostof the European Continent, leprosy still persists in a small regionon the Baltic Sea and on the Iberian peninsula. It is still present, too,in the southernmost part of the United States. \n# WHAT LEPROSY Is AND Is NoT\n\nLeprosy has plagued man for a long time.According to Dr. \nH.L.Arnold,Jr.,(Journal of the American Medical Association,May 16,1966),the disease known today by that name was firstdescribed in the literature of India and of China half a millennium \nB.C.The term leprosy,so often encountered in the Bible, does not \n1 \ndenote this disease at all,but may refer to any one of several skindiseases.Only a little more than a century ago,English-speakingphysicians called the disease leprosy,\"Greek elephantiasis,\"andapplied the term leprosy to the skin disease now called psoriasis. \nLike syphilis, malaria and tuberculosis, leprosy is a chronic in-fectious disease. The infectious agent, discovered in cutaneouslesions almost a century ago by Gerhard Hansen, and now calledMycobacterium leprae,is closely related to the bacteria that causetuberculosis. The disease is often called Hansen's disease, and thcinfective organism,Hansen's bacillus.Although this microorganismcan initiate a chronic inflammatory response in all parts of thebody,it seems to have a predilection for skin and nerves. \nBoth skin and nerves are affected in all patients, but those inwhom the nerves are chiefly involved are said to have tuberculoidleprosy. These patients often have discolored patches in the skin, and areas where sensation is impaired. Those who have thickeningand nodular swellings of the skin are said to have lepromatousleprosy,a more rapidly progressive form of the disease. In thenodules and thickened skin,Hansen's bacilli are very numerous andeasily found on microscopic exanination, but they are rarely foundin the skin lesions and affected nerves of those with tuberculoidleprosy. \n# THE AFRICAN MACULE\n\nThe tuberculoid form is much more common in Nigeria and theCongo, where, seemingly, about one in 56 persons has leprosy.Inthese regions, the prevalence of the lepromatous type, about one in560 persons,is about the same as it was in Norway in the 1840's.(See Editorial,Journal of the American Medical Association,August 15,1966.) \nAn explanation for the situation seen in the Congo and in Ni-geria was suggested by Dr.T.F.Davey(Methodist MissionarySociety,London)in The Pathogenesis of Leprosy,Ciba Founda-tion Study Group No.15.\"In our leprosy survey work in Nigeria,\"he said,\"polyneuritic leprosy has been relatively rare -certainlynot more than one per cent of all the cases seen.\"Many nerves areaffected in polyneuritic leprosy.\"The vast majority of peoplepresented skin lesions, and in survey work,as distinct from work ata centre, probably 50 per cent of those seen had a single skin lesion. In many cases this was not related to any nerve enlargement but was \nof the African macule type.”A macule is a discolored spot on theskin not raised above the surface. \n# HIGHLY
