disease(pronouncedaltz'hi-merz)is a little-known but surpris-ingly common disorder that affects thecells of the brain. It is a disease that pro-duces intellectual impairment in 500,000to 1.5million American adults. While ex-perts formerly believed that Alzheimer'sdisease occurred most often in personsunder age 65,this disorder is now recog-nized as the most common cause of severeintellectual impairment in older individ-uals. \nThe changes most commonly as-sociated with Alzheimer's disease occur inthe proteins of the nerve cells in the cere-bral cortex—the outer layer of the brain—leading to an accumulation of abnormalfibers.Under the ordinary microscopethese changes appear as a tangle of fila-ments.These neurofibrillary tangles werefirst described in 1906 by Alois Alzheimer,a German neurologist. \nNew and highly sophisticated in-struments and techniques—such as theelectron microscope which can magnifycells more than a hundred thousand times—have revealed other changes in the brainthat are characteristic of the disease.Scat-tered throughout the cortex, groups ofnerve endings degenerate and disrupt thepassage of electrochemical signals betweenthe cells. These areas of degeneration havea special appearance under the microscopeand are called plaques. The larger thenumber of plaques and tangles, the greaterthe disturbance in intellectual function andmemory. \nAt first, the individual experi-ences only minor and almost impercepti-ble symptoms that are often attributed toemotional upsets or other physical ill-nesses.Gradually,however,the personbecomes more forgetful, particularly aboutrecent events. The individual may neglectto turn off the oven, may misplace things, may recheck to see if a task was done, maytake longer to complete a chore that waspreviously routine,or may repeat alreadyanswered questions. As the disease pro-gresses,memory loss increases and otherchanges, such as confusion, irritability, restlessness, and agitation, are likely toappear in personality, mood, and behav-ior.Judgment,concentration,orientation,and speech may also be affected. In themost severe cases, the disease may even-tually render its victims totally incapableof caring for themselves. \nThere are many different pat-terns in the type, severity, and sequenceof changes in mental and neurologicalfunctioning that result from Alzheimer'sdisease. The symptoms are progressive, but there is great variation in the rate ofchange from person to person. In a fewcases, there may be a rapid decline, butmore commonly, there may be manymonths with little change.Limitations inphysical activity during the later stages \nmay cause the person to have less resist-ance to pneumonia and other physical ill-nesses that may shorten remaining lifeexpectancy by as much as one-half.Although the person with \nAlzheimer's disease may deny or be una-ware of the full extent of his or herlimitations—especially later in the courseof the illness—the inexplicable changes inessential functions are a source of deepfrustration for those afflicted and for theirloved ones. \n# How is the diagnosis made?\n\nBefore a diagnosis of Alzheimer'sdisease is made, other illnesses which maycause memory loss must be excluded.Thecondition must be differentiated from themild and occasional forgetfulness thatsometimes occurs during normal aging.Depression, which is fairly common in el-derly individuals facing a variety of stress-ful situations, may also affect memory. \nApproximately one-half of el-derly men and women with severe intel-lectual impairment are victims ofAlzheimer's disease.About another fourthof the overall group suffer from vasculardisorders, especially multiple strokes, andthe balance have a variety of other con-ditions;for example, brain tumors,abnor-mal thyroid function, infections, perniciousanemia, adverse drug reactions, and ab-normalities in the spinal fluid sy","cbCaivVoQk11TKxq","https://ap.wps.com/l/cbCaivVoQk11TKxq","pdf",406243,16,"English","# What is
