A cross-sectional parent survey was conducted in a closed model HMO among parents of 19–35 month old children. Factor analysis confirmed sub-domains and Cronbach’s alpha evaluated internal consistency; construct validity used linkage to children’s immunization records. Results: 46% response (N=230), three factors (70% variance) and a revised 15-item instrument with alphas .74, .84, .74; higher PACV scores corresponded to more under-immunization.
