performapproximately 600,000 hysterectomies eachyear. This rate has remained stable for thepast 30 years, despite advances in tech-niques such as endometrial ablation formenorrhagia and fibroids—two of the mostfrequent conditions treated with hysterec-tomy.'Women between the ages of 40 and44 years have a higher rate of hysterec-tomies than any other age group, althoughrates among women aged 45 to 54 yearshave increased from 8.9 per 1000 women in1994 to 10 per 1000 in 1999. \nUterine leiomyoma, endometriosis, anduterine prolapse precipitate most hysterec-tomies.To treat these conditions, physiciansgenerally rely on total abdominal hysterec-tomy(TAH),total vaginal hysterectomy(TVH),and laparoscopic-assisted vaginalhysterectomy(LAVH),all of which involveremoval of the cervix.Because hysterec-tomies continue to be necessary, interest hasgrown in less invasive laparoscopic tech-niques.While surgeons have adoptedlaparoscopic approaches for other gyneco-logic procedures, many have not yet madethis transition for abdominal hysterectomy \nEmerging in 1991,the laparoscopic supra-cervical hysterectomy (LSH)is the onlylaparoscopic procedure that aims to retainthe cervix.Advances in laparoscopic equip-ment have made the procedure more readilyaccessible and acceptable to gynecologists. The concept of conserving the cervix was notnew, however. Before 1950,almost all hys-terectomies were performed abdominallywith a supracervical approach. This routewas preferred because it avoided entry intothe vagina, shortened the duration of anes-thesia,and reduced the risk of surgicalcomplications. The approach fell out offavor as surgical training and anesthetictechniques improved, making total hysterec-tomy the standard of care in the latter part ofthe 20th century. \nToday,as awareness of LSH grows,so does its popularity amonggynecologists.Advocates of LSH cite its benefits²:The
