Their composition typically changes in response to the metaplastic sequence triggered by Helicobacter pylori infection. This alteration in gastric mucins is also observed in cases of gastric cancer, although the precise connection between mucin expressions and gastric carcinogenesis remains uncertain. This review first introduces the relationship between mucin expressions and gastric metaplasia or cancer observed in humans and mice. Additionally, we discuss potential pathogenic mechanisms of how aberrant mucinsand their glycans affect gastric carcinogenesis. Finally, we summarize challenges to target tumor-specific glycans by utilizing lectin-drug conjugates that can bind to specific glycans. Understanding the correlation and mechanism between these mucin expressions and gastric carcinogenesis could pave the way for new strategies in gastric cancer treatment. \nK E Y WO R D S \ngastric cancer, gastric mucin, glycans, Helicobacter pylori, intestinal metaplasia \n1 | INTRODUCTION \nMucins represent high-molecular-weight epithelial glycoproteins characterized by a dense concentration of oligosaccharides linked O-glycosidically to tandem repeat peptides abundant in threonine, serine, and proline, which are vital in maintaining gut homeostasis. They form a gel-like mixture, protecting gastrointestinal health. Especially positioned at the gastric interface, mucin glycoproteins safeguard epithelial cells from digestive enzymes and acidity in the \ngastric lumen, affect epithelium lubrication, and regulate molecular pathogenesis. Moreover, mucins act as receptors, binding molecules, interacting with pathogens, and modulating inflammation by sensing external stimuli to trigger intracellular pathways.1 \nVarious types of mucins and mucin-related glycans are expressed across different organs, and alterations in mucin expression often occur within the carcinogenic process.2 These changes occasionally reflect the development of cancer and can assist in categorizing cancer subtypes, such as the gastric/intestinal types of gastric cancer \nAbbreviations: GC, gastric cancer; H. pylori, Helicobacter pylori; IM, intestinal metaplasia. \nThis is an open access article under the terms of the Creative Commons Attribution-NonCommercial License, which permits use, distribution and reproduction in any medium, provided the original work is properly cited and is not used for commercial purposes. \n© 2024 The Author(s) .
