South Africais among the United Nations member states thatcommitted to the agenda to improve the quality oflife of all our citizens. SA is also a signatory to otherimportant local and international Declarations onHIV and TB andit is time for ordinary South Africansto experience the full benefit of these commitments. The country’s commitments for improving people’slives are further demonstrated in our nation’s ownNational Development Plan (NDP) 2030 andtheNSP is a strategic framework that guides the multi -stockholder HIV, TB and STIs contribution towards theNDP, and ultimately Agenda 2030. \nThis particular NSP is the last one to see the countrythrough the 2030 finish line. It is therefore importantto ensure that itis guided by hard evidence informedby global trendsand emerging priorities, but alwaysintentionally taking the country’s epidemiologicalprofile into context, as we address the threeepidemics. This NSP has also been informed bylessons learnt from the COVID-19 pandemic whichcaused significant disruptions of access to HIV, TBand STIs prevention, screening, testing and treatmentservices. The disruptions resulted in the defermentof the 2017-2022 NSP by an additional year in orderto implement Catch-Up Plans to mitigate against thenegative impact thereof. The proper implementationof the new NSP will require strong political will, robustcivil society and private sector involvement, efficientgovernment services, as well as a coordinatedsupport by development partners and, above all,an active participation by all South Africans. In brief,we will only get it right with shared leadership andshared accountability among all stakeholders. \nAlthough we have made considerable stridesincertain areas, such as HIV-testing, we are still laggingbehind on initiating and retaining infected peopleon antiretroviral therapy. Among the nearly 8 millionpeople living with HIV, about 5,7 million of them areon treatment, leaving us with a 5.7 gap of over \n2 million people who should be on treatment butare not. It is also concerning that the percentage andlevels of viral suppression are not enough to reap thefull benefits of treatment as prevention. There aresimilar challenges with TB – the data indicates thatin 2021, South Africa had aTB incidence of 304 000,however, only 187 375 notifications were recorded, thus leaving us with a gap of 111 625 missing peoplewith TB. We must work harder to scale up awareness, screening and testing, treatment adherence and tocombat TB-related stigma and discrimination. Thereis also an increasing trend of people infected with TBwho are asymptomatic and are not picked up withroutine screening – this calls for an urgent move toutilise advanced testing tools such as digital chestX-rays, which are game changersin the detectionof asymptomatic TB. Involvement of communitiesremains a key factor in managing and combating TBto minimise the development of MDR/XDR TB. \nAs we begin the implementation of this NSP until2028, it cannot be business-as-usual. We must findinnovative ways of providing quality health servicesto people and we must ensure that our health \n/ iii \nNational Strategic Plan for HIV, TB and STIs, 2023-2028 \nPreface \nsystems are resilient and responsive.
