The term "School Age Child Care Programs" is defined as follows:
"School age programs provide child care services exclusively for school age children, ages 6
through 12. Curriculum, activities, and staff training are focused on the needs of school age children. These programs do not provide care for infants, toddlers, or preschool children, unless programs housed in public school facilities provider services to kindergarten and prekindergarten age children. (See * below.)
Care is provided for children ages 6 through 12 after regular school days, on school holidays, and during summer vacation. Space available for programs is often a public school cafeteria, making the physical environment different from other child care being assessed.
Examples of these kinds of programs are YMCA or YWCA multiple facility programs, or
programs provided by independent school districts for after-school or holiday care. "
*This packet includes only school age measures. When scoring school age programs that include kindergarten (age 5) and/or kindergarten (age 4) children, the measures that are most appropriate for the preponderance of children in the group should be used. Every child must have supplies, curriculum, activities, and adult interactions that are developmentally appropriate, and the assessment instrument must reflect this attention to individual needs. Assessors may review measures in the preschool assessment packet to score the applicability of activities and materials for 4 and 5 year old children, but the assessment instrument in this packet must be used.
Additional assessment packets are available for scoring licensed child care centers serving preschool children only (ages infants through 5 years of age) and centers serving all ages of children. Scoring and compliance notes throughout the assessment instruments give information regarding the use of each instrument.
TEXAS RISING STAR PROVIDER CERTIFICATION GUIDELINES
ASSESSMENT RESULTS
Name of Facility:
Address:
Street City Zip
Director: Telephone Number ( ) Provider Number: Agreement Effective Date: Licensed Capacity: Agreement Termination Date: Age Range Served: From: Through: Number to be Served: OR (check if appropriate) as space is available.
ASSESSMENT RESULTS
Based on the results of this assessment the following action was taken:
The facility received final approval on , to expire on
date date
The facility received provisional approval on: , to expire on
date date
Service Improvement Agreement issued, dated: , Improvements Completed:
date
Deferred Status: Service Improvement Agreement was issued, dated:
Facility Completed Improvements: yes no
Further action required by facility: Waivers issued: (1) Expiration Date:
(2) Expiration Date:
Assessed by: Assessment Completed:
date
Date discussed with Provider: Provider Signature:
SUMMARY OF TEXAS RISING STAR PROVIDER ASSESSMENT SCORING Name of Facility:
USE WITH ASSESSMENT FOR SCHOOL-AGE PROGRAMS

| | | Four-Star
Certification | Three-Star
Certification | Two-Star
Certification | Deferred Status
(Not Certified) | Facility
Score | Group scores | | | | | |
| --- | --- | --- | --- | --- | --- | --- | --- | --- | --- | --- | --- | --- |
| Measure | Scoring Range | | Scoring Range | Scoring Range | Minimum score | Score
one time | Score one time per group.
