Survival curves were generated using the Kaplan–Meier method and comparisons were made using the log-rank test. Univariate and multivariate Cox proportional hazards regression analyses were employed to identify factors independently associated with five-year event-free survival (EFS) . \nResults A total of 319 elderly patients with stage I-III breast cancer were included, with a 5-year overall EFS rate of 82. 0% . Univariate analysis revealed significant associations between 5-year EFS and age, histological grade, estrogen receptor (ER) status, progesterone receptor (PR) status, Ki-67 level, tumour size (T stage), lymph node status (N stage), and the administration of endocrine therapy (all P \u003C 0. 05) . Multivariate analysis revealed that age, PR status, T stage, N stage, and Ki-67 level were independent prognostic factors for 5-year EFS. \nConclusions The results of this study suggest that age, PR status, the Ki-67 index, tumour size, and lymph node metastasis can serve as independent predictors of the 5-year EFS rate in elderly patients with stage I-III breast cancer. Specifically, younger age (≤ 68 years), PR positivity, Ki-67 \u003C 30%, and early T (T1-T2) and N (N0) stages are associated with a more favourable prognosis. \nKeywords Elderly breast cancer patients, Event-free survival, Survival prognosis, Endocrine therapy \n*Correspondence: \nPurong Zhang [zprrong123321@163.com](zprrong123321@163.com)[ ](zprrong123321@163.com)Yuying Wang [jyawyy202@163.com](jyawyy202@163.com) \n1School of Medicine, University of Electronic Science and Technology of China, Chengdu, Sichuan 611731, China \n2Department of Breast, Sichuan Clinical Research Center for Cancer, Sichuan Cancer Center, Sichuan Cancer Hospital & Institute, University of Electronic Science and Technology of China, Chengdu, Sichuan, China 3Department of Health Management Center, Qionglai Medical Center Hospital, Qionglai, Sichuan Province, China \n4North Sichuan Medical College, Nanchong, Sichuan Province, China \nIntroduction \nAccording to survey data from the International Agency for Research on Cancer (IARC) in 2022, the incidence rate of female breast cancer (11.6%) ranks second among newly diagnosed cancers, and it shows a continuously increasing trend [1]. As a complex and highly heterogeneous disease, the treatment and prognosis of breast cancer are highly correlated with various factors, such as tumour subtype and stage. The 2023 version of the National Comprehensive Cancer Network (NCCN) Clinical Practice Guidelines in Oncology for Older Adults defines the elderly population as individuals aged 65 years and above [2]. \n© The Author(s) 2026.
