Inflammation‑based biomarkers have emerged as potential predictors of survival in various cancers. However, their prognostic role inoCRC remains unclear. \nPatients and Methods: This retrospective study included patients who underwent surgery for histopathologically confirmed oCRC at Sakarya University Training and Research Hospital between January 2015 and February 2024. Preoperative systemic inflammatory markers–C‑reactive protein‑to‑albumin ratio (CAR), lymphocyte‑to‑C‑reactive protein ratio (LCR), prognostic nutritional index (PNI), neutrophil‑to‑lymphocyte ratio (NLR), platelet‑to‑lymphocyte ratio (PLR), systemic inflammatory index (SII), and HALP score–were analyzed. Overall survival was recorded, and statistical analyses were performed. \nResults: Among the inflammatory markers, CAR, LCR, and PNI were significantly associated with overall survival at 1, 3, and 5 years (p \u003C0.05). LCR demonstrated the highest sensitivity and specificity in predicting mortality. Patients with higher CAR and lower PNI values had significantly poorer outcomes. \nConclusion: Preoperative systemic inflammatory markers, particularly LCR, CAR, and PNI, are valuable prognostic indicators in patients with oCRC. These easily obtainable markers may help guide clinical decision‑making and improve individualized patient management. \nKeywords: Obstructive colorectal cancer, ileus, systemic inflammatory markers, survival, prognostic factors. \n✉Ahmet Tarik Harmantepe, Sakarya University Training and Research Hospital, Turkey. Anticancer Research is published by the International Institute of Anticancer Research. \nIntroduction \nColorectal cancer (CRC) is the third most commonly diagnosed cancer and ranks second in cancer‑related mortality worldwide (1) . It has been reported that approximately 8% to 29% of patients diagnosed with CRC develop acute bowel obstruction either at the time of diagnosis or during disease progression (2, 3). Compared to non‑obstructive CRC cases, patients with obstructive CRC (oCRC) are often diagnosed atmore advanced clinical stages and demonstrate a higher risk of recurrence. \noCRC is associated with higher surgical mortality ratesand poorer prognoses (4) . Managing advanced CRC becomes more challenging due to complications such as electrolyte imbalances, dehydration, sepsis, or perforation (4). Postoperative morbidity and mortality rates for oCRC have been estimated at approximately 40% and 8% to 15%, respectively, with an overall survival rate of only 12% to 31%(3, 5). \nSystemic inflammatory responses associated with cancer are critical indicators of tumor progression. Inflammation plays a significant role not only within the local tumor microenvironment but also systemically, influencing tumor biology (6) . These responses often involve changes in the secretion of cytokines, hormones, growth factors, and acute‑phase proteins (7‑9). \nRecent studies have highlighted that biomarkers such as C‑reactive protein (CRP), complete blood count ","cbCainNgaSm8MZy4","https://ap.wps.com/l/cbCainNgaSm8MZy4","pdf",745000,"English","# Introduction\n# Patients and Methods\n# Results\n# Conclusion","[{\"question\":\"What is the main purpose of the study on obstructive colorectal cancer?\",\"answer\":\"To evaluate prognostic factors that can predict survival outcomes in patients with obstructive colorectal cancer, focusing on systemic inflammatory markers.\"},{\"question\":\"Which preoperative inflammatory markers were analyzed?\",\"answer\":\"CAR, LCR, PNI, NLR, PLR, SII, and the HALP score were analyzed, alongside overall survival outcomes.\"},{\"question\":\"What markers were most strongly associated with overall survival?\",\"answer\":\"CAR, LCR, and PNI were significantly associated with overall survival at 1, 3, and 5 years, and LCR showed the highest sensitivity and specificity for predicting mortality.\"}]","Survival Predictors in Obstructive Colorectal Cancer - A Combined Clinical, Inflammatory, and Histopathological Approach | PDF",1790192500,25]
