They are typically asymptomatic but can cause various complications and even sudden death. Management typically involves surgical resection and close monitoring, but clear indications for surgery are not well established. \nCase summary A 67-year-old Chinese female, with a history of cervical cancer surgery, was found to have a right atrial mass during routine chest computed tomography follow-ups. The patient was asymptomatic, although an electrocardiogram showed incomplete right bundle branch block. Over several years, the mass increased in size. Imaging studies revealed a well-defined, non-avid, fat-containing right atrial mass. The patient underwent successful surgical excision of the tumor, and postoperative pathology confirmed it was a lipoma. Her recovery was uneventful, and the incomplete right bundle branch block resolved. \nConclusions Cardiac lipomas require individualized evaluation and management based on their size, location, and potential to cause complications. Immediate surgical intervention is not always necessary, particularly forasymptomatic cases. However, careful long-term follow-up is crucial, with surgery considered when there is evidence of tumor growth, arrhythmias, or other risk factors for complications. \nKeywords Cardiac mass, Benign cardiac tumor, Cardiac lipoma, Surgery \n†Zhen Wang and Haoyang Li contributed equally to this work. \n*Correspondence: Hongfei Xu [xuhongfei@zju.edu.cn](xuhongfei@zju.edu.cn)[ ](xuhongfei@zju.edu.cn)Weidong Li[liweidong@zju.edu.cn](liweidong@zju.edu.cn) \n1Department of Cardiovascular Surgery, School of Medicine, The First Affiliated Hospital of Zhejiang University, No. 79 Qingchun Road, Hangzhou, China \nIntroduction \nCardiac lipomas are rare, benign, encapsulated tumors primarily composed of mature adipose tissue [1]. Their incidence is less than 0.1% in the general population [2], accounting for approximately 8.4% of all primary cardiac tumors [1]. The clinical presentation and potential complications of cardiac lipomas largely depend on their size and location. While often asymptomatic, they can cause symptoms such as chest discomfort, arrhythmias, syncope, or even sudden death [3]. Imaging modalities such as echocardiography, computed tomography (CT), and magnetic resonance imaging are generally useful in aiding the diagnosis of cardiac lipomas, but their management remains a subject of debate.
