However, their use in advanced-age patients remains insufficiently studied. Case Presentation: A 90-year-old man with a history of wide excision for cutaneous melanoma developed liver metastases. Core needle biopsy confirmed the presence of a BRAFV600E mutation. The patient was treated with dabrafenib (150 mg twice daily) and trametinib (2 mg once daily). After 6 months, he achieved complete remission of liver metastases, experiencing only mild adverse events, including grade 1 pyrexia and diarrhea. Conclusion: This case report demonstrates that dabrafenib-trametinib combination therapy is an effective and well-tolerated treatment option for BRAF-mutated metastatic melanoma in advanced-age patients. \n© 2025 The Author(s) . \nPublished by S. KargerAG, Basel \nIntroduction \nHistorically, the prognosis for patients with metastatic melanoma has been poor, with a median overall survival of \u003C1 year [1, 2] . However, advancements in targeted therapies have markedly improved clinical outcomes, particularly in individuals with BRAF V600E mutations. \nCorrespondence to: \nSang-Bo Oh, sangboimda [hanmail.net](hanmail.net) \nCase Reports in Oncology \nCase Rep Oncol 2026;19:34–38 35 \n\n| DOI: 10. 1159/000549686 | © 2025 The Author(s) . This dual-agent regimen has demonstrated significant survival benefits in patients with BRAF-mutant metastatic melanoma while maintaining a manageable toxicity profile [4] . Despite these advantages, data on the use of this regimen in the older population remain limited. Given the increasing incidence of melanoma among older adults, evaluating the safety and efficacy of this therapeutic approach in this population is essential. However, for patients aged 85 years and older, these reports are exceedingly rare, with few case reports describing adverse events such as renal impairment [5] . No large-scale studies specifically addressing this very elderly subgroup have been published. This report presents the case of a super-elderly patient with metastatic melanoma harboring a BRAF mutation who achieved a complete response following treatment with dabrafenib and trametinib. \nCase Presentation \nAn 89-year-old Korean man was diagnosed with malignant melanoma of the posterior neck in August 2016.
