systems in at leasteight states have put into place for-mal policies and procedures for thediagnosis and treatment of the hepa-titis C virus(HCV). \nMost HCV guidelines are basedon the National Institutes of Healthrecommendations and currenttreatment literature, and place aheavy emphasis on exclusionary cri-teria,prohibiting treatment for a va-riety of reasons. \nCorrectional Physician Servicesand two other vendors in Pennsyl-vania adopted the policies.Otherlarge state correctional systems thathave taken formal positions includeCalifornia,Florida,New York,Texas, and Virginia. \nSome of the smaller state sys-tems—Connecticut and Rhode Is-land,for example—also have estab-lished guidelines,in an attempt tomanage their burgeoning HCVcaseloads. \nIn most states, the HCV guide-lines contain length of stay criteria, sanctioning treatment only for in-mates with at least 15 to 18 monthsleft on their sentences. \nFor the most part, the guidelinesare an attempt to place HCV in itsrightful place among competinghealth care demands and needs.Theguidelines determine who is a can-didate for HCV treatment, who is \ntreated and ultimately how many re-sources are spent on the afflictionwithin correctional systems. \nIn Rhode Island, about one thirdof the state's combined jail andprison population of 3,100 haveHCV.By the end of 1999,onlyabout 30 inmates qualified fortreatment under the department ofcorrection's HCV guidelines, whichhas,in turn, made treatment costsaffordable. \n\"If you prioritize who you aretreating and don't just treat peoplewho are passing through,it is reallya lot more affordable than when youfirst think about the problem,\"aRhode Island health official said.\"The key is to treat people who arewith you for longer periods oftime.\" \nIn New York, the state Depart-ment of Correctional Services hasadopted a similar approach, puttingin place clinical care guidelines thathave drastically reduced the numberof inmates who qualify for treat-ment.The department is treatingabout 150 inmates with HCV,asmall percentage of those who havethe disease. \n\"If a system does not haveclinical care guidelines, they are atthe mercy of anyone they happen tobe using as a consultant that week,”says Lester Wright,MD,Associate \nCommissioner and Chief MedicalOfficer for the New York State De-partment of Correctional Services.“When that happens, you end upwith inconsistent care.” \nThat last point has becomereadily apparent to the WashingtonState Department of Corrections.Ithad been handling HCV on a case-by-case basis, using an \"outdatedpolicy\"from the mid-1990s to maketreatment decisions, says AdamJonus,MD,Medical Director forHealth Services for the state's cor-rections department. The depart-ment adopted a new policy July 1. \nAbout 700 inmates have testedpositive for the HCV antibodies, themajority of whom probably haveHCV disease, according to Jonus. \nIt is a good idea to look at hepa-titis C from\"ground zero,\"saysDavid Thomas,MD,director ofHealth Services for the Florida De-partment of Corrections.About 4million people
