The program also partners with faith congregations through a Community Resource Liaison to assess needs, support advance care planning conversations, and connect residents with resources, while maintaining a local, independent, non-profit governance model.","GRIEF SUPPORT  \nRainbow Grief Support is one of the most important services we provide. Our bereavement counselors work closely with anyone who has experienced the loss of a loved one, whether on our hospice program or not.  \nTheir outreach takes many forms including phone calls, one-on-one visits, facilitation of monthly or weekly support groups like Monday Morning Joe and hosting special programs like our annual Memorial Service or Holiday Grief events.  \nIn 2022, we dedicated a beautiful, newly designed Memorial Garden outside our inpatient center.  \nAnyone is welcome to visit the outdoor space to remember and honor the important people in their lives and gather for healing, reflection, and solitude.  \nCOMMUNITY HEALTH MINISTRY Rainbow Community Health Ministry is a program dedicated to collaborating with faith congregations to enhance the overall well-being of the community. Our  \nCommunity Resource Liaison supports parish nurses, most of whom work on a volunteer basis, by completing tasks outside their area of expertise (e.g., conducting needs assessments, facilitating advance care planning conversations) and connecting parishioners with programs and resources that helpmeet critical needs.  \nFRIENDS IN ACTION Rainbow Friends in Action is a volunteer program dedicated to providing “neighborly assistance” to  \npeople who are elderly, disabled or seriously ill and could benefit from receiving a little extra non-medical help in order to remain living independently in their homes.  \nCore services provided by Rainbow Friends in Action include:  \n• Friendly visits  \n• Shopping and errands  \n• Transportation to and from medical appointments, the pharmacy, church, etc.  \nWe review other services on a case-by-case basis such as light housekeeping, meal preparation, and yard work (e.g., mowing, raking, snow shoveling).  \nRAINBOW COMMUNITY CARE IS LOCAL, INDEPENDENT & NON-PROFIT  \nLocal – serving Dodge and Jefferson counties as well as portions of Columbia, Dane, Rock, Walworth and Waukesha counties.  \nIndependent – we don’t employ anyone or own anything that is required to use us. These wishes are put into a legal document known as an advance directive. It helps others know what type of medical care you want, allows you to express your values and name a healthcare agent (someone who can make decisions for you if you cannot do so yourself).  \nGood advance care planning starts with an important conversation with your loved ones about your healthcare wishes and is revisited when life circumstances change. Appropriate times to have this talk include when you begin each new decade of life, you are diagnosed with a serious illness, a loved one or health care agent passes, you get divorced, or you can no longer live on your own.  \nRainbow currently participates in a statewide initiative called Advance Care Planning & Advance Directive by the Wisconsin Medical Society.
