However, immunotherapy has minimal benefit for patients in most types of cancer and is largely ineffective in some cancers (such as pancreatic cancer and glioma) . A synergistic anti-tumor response may be produced through the combined application with traditional tumor treatment methods. Radiotherapy (RT) not only kills tumor cells but also triggers the pro-inflammatory molecules’ release and immune cell infiltration, which remodel the tumor microenvironment (TME) . Therefore, the combination of RT and immunotherapy is expected to achieve improved efficacy. In this review, we summarize the effects of RT on cellular components of the TME, including T cell receptor repertoires, different T cell subsets, metabolism, tumor-associated macrophages and other myeloid cells (dendritic cells, myeloid-derived suppressor cells, neutrophils and eosinophils). Meanwhile, non-cellular components such as lactate and extracellular vesicles are also elaborated. In addition, we discuss the impact of different RT modalities on tumor immunity and issues related to the clinical practice of combination | |\n| --- | --- | --- |\n| | | |\n| | | |\n| | | |\n| | | |\n| | | |\n| | | |\n| | | |\n| | | |\n| | | |\n| | | |\n| therapy. | | |\n| | Cell Death and Disease (2023)14:679; [https://doi.org/10.1038/s41419-023-06211-2](https://doi.org/10.1038/s41419-023-06211-2) | |\n| | | |\n\nFACTS \n● Cancer immunotherapy plays an important role in tumor treatment strategies. \n● Objective tumor response rates and treatment sensitivity of cancer immunotherapy are still poor. \n● The complicated and dynamic tumor microenvironment influences the efficacy of immunotherapy. \n● Radiotherapy remodels the tumor microenvironment by affecting multiple cellular and non-cellular components. \nOPEN QUESTIONS \n● Can radiotherapy improve the efficacy of cancer immunotherapy by remodeling the tumor microenvironment? \n● What are the specific mechanisms that radiotherapy remodels the tumor microenvironment? \n● Does radiotherapy combined with immunotherapy have the potential for clinical application to help patients? \nINTRODUCTION \nCancer immunotherapy (CIT) is a treatment method that stimulates the immune system to attack and suppress tumor development, including promoting immune activation and relieving immune suppression [1] . This treatment has shown unprecedented response in patients and brought hope to many cancer patients. This strategy includes immune checkpoint blockade (ICB), adoptive cell therapy (ACT), cancer vaccines, and other approaches that target specific molecules or pathways involved in immune suppression or activation. Currently, the most widely studied and applied immunotherapy is ICB targeting immune checkpoint (IC) molecules such as programmed cell death 1 (PD- 1), programmed cell death ligand 1 (PD-L1) and cytotoxic T lymphocyte-associated protein 4 (CTLA-4) . Although immunotherapy has demonstrated significant efficacy in clinical applications, only a small proportion of patients benefit, with an objective response rate of 10–30%[2] . The implementation of CIT requires a thorough understanding of the interaction between the tumor and the immune system and the intricate regulatory networks in the tumor microenvironment (TME) . \n1Department of Colorectal Surgery, The First Affiliated Hospital of Zhengzhou University, 450052 Zhengzhou, Henan, China. 2Henan Institute of Interconnected Intelligent Health Management, The First Affiliated Hospital of Zhengzhou University, 450052 Zhengzhou, Henan","cbCaibc3Ru0EfvuX","https://ap.wps.com/l/cbCaibc3Ru0EfvuX","pdf",2993508,"English","# Facts\n# Open Questions\n# Introduction\n# Radiotherapy-induced immunomodulation and TME remodeling\n## Effects on immune cell components\n## Effects on non-cellular components\n# Clinical implications of combining RT with immunotherapy","[{\"question\":\"Why does cancer immunotherapy benefit only a small proportion of patients?\",\"answer\":\"Despite major advances, many patients show poor objective response rates.
