the staff, residents, nurse anes-thetists and nurse students on a bi-monthly basis formore than 10 years. They were grossly underreporteddue to personnel's unwillingness to disclose such theadverse events.Using this incomplete data as theindicators for quality assessment may lead to inappro-priate prioritization of the problems and qualityimprovement of the service. \nSince 2000,our department has applied theconcept of risk management which is one of the keyelements of hospital accreditation to improve thequality of patient's care. This has motivated allpersonnel in the department to recognize the impor-tance of risk identification and assessment as a partof risk management and led to the establishment ofa new quality assessment system starting on May \n1,2001.By selecting the key indicators with clearobjectives emphasizing quality improvement of theservice and with the agreement and cooperation ofthe personnel,we have set up a simple and precisesystem for quality assessment. \nThe main objective of this system is to assessthe quality of all anesthesia services in all patientswho receive anesthetic care at Siriraj Hospital byprospectively studying the incidences of seriousanesthetic complications and patient's satisfactionof the anesthetic care. The main indicators include \n1)Mortality rate during anesthesia and within 24hours,2)Cardiopulmonary resuscitation(CPR)rateduring anesthesia,3)Major complications during anes-thesia,4)Drug mishaps and adverse drug reactions, \n5)Anesthetic equipment related complications, and \n6)Patient's satisfaction. \n# METHOD\n\nWe prospectively collected the data of allpatients who received anesthetic care from all servicesat Siriraj Hospital during May-December 2001.Thestudy was comprised of two main steps. \ncomplicated because most significant incidences occurring during anesthetic care have been recognizedand solved without any residual detectable signs
