Locally aggressive lung tumors have the potential to extend into the left atrium via pulmonary veins, which can further complicate by embolizing into the systemic circulation. Pulmonary blastoma (PB) is one of the rare forms of primary lung malignancy and is locally aggressive. We report a rare case of 30 years old male patient who underwent left pneumonectomy for PB. During resection, the tumor was embolized into the descending thoracic aorta, leading to an acute circulatory compromise of both the lower limbs. \nKeywords: Embolization, pulmonary blastoma, pulmonary vein, transesophageal echocardiography \nAddress for correspondence: Dr. Krishna Prasad Gourav, Room No 4014, Anesthesia Office, Level 4, Advanced Cardiac Center, PGIMER, Chandigarh, India. \nE‑mail: [kpgmbbs777@gmail.com](kpgmbbs777@gmail.com) \nSubmitted: 18‑Jul‑2023 Revised: 15‑Aug‑2023 Accepted: 30‑Aug‑2023 Published: 12‑Jan‑2024 \nINTRODUCTION \nLung cancer metastasizes to the heart in around 25% of cases. [1] Primary pulmonary malignancy with left atrial extension via the pulmonary veins has been well documented. [1‑3] Tumor can even embolize into the systemic circulation and can cause further complications. [4] Pulmonary blastoma (PB) is one of the rare forms of primary lung malignancy and is locally aggressive. [5,6] Wereport a case of 30 years old male who underwent left pneumonectomy for PB. In the immediate post‑operative period, the patient developed acute circulatory compromise of lower limbs due to tumor embolism into the descending thoracic aorta (DTA) . We hereby discuss a few clinical pearls related to the management of this case. Written consent has been obtained from the patient. \nCASE PRESENTATION \nA 30‑year‑old male patient presented to our tertiary care institute with the chief complaint of cough with \n\n| Access this article online | |\n| --- | --- |\n| Quick Response Code: | Website:\u003Cbr>[https://journals.lww.com/aoca](https://journals.lww.com/aoca) |\n| | |\n| | DOI:\u003Cbr>10.4103/aca.aca_115_23 |\n\nexpectoration for 4 months and was diagnosed to have a PB in the upper lobe of the left lung. He was a chronic smoker and alcoholic for the past 10 years. The chest radiograph (postero‑anterior view) showed homogeneous opacification over the left lung field [Figure 1] . Positron emission tomography (PET) scan revealed a mass of 11 × 11 × 16 cm3 in the left upper lobe, abutting to the intercostal muscles antero‑laterally and the pericardium medially, causing mediastinal shift to the right. Pre‑operative transthoracic echocardiography (TTE) revealed no tumor extension into the heart. Left pneumonectomy was planned after thorough pre‑anesthetic evaluation. \nOn the day of surgery, compliance with implementation of the World Health Organization (WHO) surgical safety checklist was ensured. After applying American Society of Anesthesiologists (ASA) standard monitoring, a 16 G intravenous cannula was inserted on the dorsum of the left hand and right radial artery cannulation was done with a 20 G cannula to monitor invasive blood \nThis is an open access journal, and articles are distributed under the terms of the Creative Commons Attribution‑NonCommercial‑ShareAlike 4.0 License, which allows others to remix, tweak, and build upon the work non‑commercially, as long as appropriate credit is given and the new creations are licensed under the identical terms. \nFor reprints [contact:](contact: WKHLRPMedknow_reprints@wolterskluwer.com)[ WKHLRPMedknow_reprints@wolterskluwer.com](contact: WKHLRPMedknow_reprints@woltersk","cbCaiucVv0BR8IH9","https://ap.wps.com/l/cbCaiucVv0BR8IH9","pdf",1110461,"English","# Abstract\n# Introduction\n# Case Presentation","[{\"question\":\"Why can lung tumors cause serious cardiac and systemic complications?\",\"answer\":\"Locally aggressive lung tumors can extend into the left atrium via pulmonary veins and may embolize into systemic circulation, leading to severe complications.\"},{\"question\":\"What happened during surgery in this reported case?\",\"answer\":\"During resection after left pneumonectomy, the tumor contents embolized into the descending thoracic aorta, producing acute circulatory compromise affecting both lower limbs.\"},{\"question\":\"What diagnostic role did echocardiography play before pneumonectomy?\",\"answer\":\"Pre-operative transthoracic echocardiography showed no tumor extension into the heart.\"}]","Paraplegia after Pneumonectomy - A Rare Complication | PDF",1790026667]
