PrEP use and adherence during pregnancy and the postpartum period in Ugandan women \nEmily Santos, B.A. \nIntroduction \nPrEP use and adherence during pregnancy \nand postpartum \n• Vulnerability to HIV increases during pregnancy and postpartum, yet little is known about HIV preexposure prophylaxis (PrEP) used at these times. \nObjective \nTo describe adherence trends and characteristics of pregnant and postpartum women using oral PrEP in Uganda to identify opportunities to reduce \nHIV incidence. \nBackground \nAbout PACO (Parent Study) \n• PACO = Placentas, Antibodies, and Child Outcomes \n• Prospective longitudinal birth cohort study of pregnant people with HIV (PPHIV), comparators without HIV, and people taking PrEP in pregnancy \n• Enrolled December 2019 – November 2024 \n• 775 pregnant people and the babies born to them (as dyads) \n• Dyads are followed through child age 5 years (every 3 months) to measure child growth, development, health outcomes, and relate them to placental findings. \nPACO (parent study) population \nIncluded: \n• Maternal age ≥18 years \n• All individuals taking PrEP in pregnancy \n• All individuals living with HIV taking ART in pregnancy \n• HIV-uninfected individuals (enrolled as the very next eligible person) \nExcluded: \n• Known or suspected multiple gestation pregnancy \n• Placenta not collected \n• Not available by telephone for post-discharge contact \n• Inability to speak English or Runyankole well enough to provide informed consent \nOur sub-study included: \n• All HIV-uninfected individuals enrolled in the parent study (PrEP users and non-users) \nData collection \nBaseline: Follow-up: \n• at enrollment during pregnancy \nAt enrollment \nAt antenatal care visits \n After delivery, before discharge \nQuestionnaire Questionnaire Questionnaire \n• after delivery before discharge \n• in-person visits every 3 – 6 months through 60 months postpartum \nEvery visit \nQuestionnaire \nTotal follow-up visits: 5,403 \nFollow-up visit attendance \n\n| 12 months | 24 months | 36 months | 48 months |\n| --- | --- | --- | --- |\n| 94% | 98% | 93% | 86% |\n\nQuestionnaire \n• Reasons for taking PrEP, perception of risk \n• Timing and reasons for stopping PrEP, if ever stopped \n• How they took PrEP: \n• Number of times/day, days/week, missed doses \n• Self-perceived adherence and protection against HIV \n• Percentage of days taken PrEP in the last month \nAnalytic methods \n• Calculated asset index (a measure of wealth) using principal component analysis \n• Used chi-square and t-tests to compare self-reported adherence to daily PrEP in pregnancy and postpartum \n• Used logistic regression models to determine associations with daily PrEP use 1) during pregnancy and 2) postpartum. \n• Variables considered for inclusion were parity, number of antenatal care visits, marital status, \nemployment, residence in Mbarara (semi-urban), and self-reported PrEP adherence during pregnancy (for postpartum model) \n• Except for age, only variables with P-value \u003C0.2 on bivariate analysis against the outcome were included
