admitting elderly people to care are usuallyinvolved in finding a balance between conflicting needs and wishes. As theWilliams Report suggested, residential care is likely to be seen as second best bythe person who is being admitted, however comfortable the home,or howeverwell the admission is managed. The notes that follow are offered as guidance tothe social worker involved in providing care for the elderly person. \n## Reasons for admission\n\nA request for help may take many forms, from an apparently straightforwardrequest for grandma to be looked after while the family takes a holiday in thesummer,to a series of demands from neighbours, doctor, and health visitor, thatan old lady must be removed from appalling circumstances immediately.Inwhatever way the situation is presented the social worker's first task is to collectthe relevant facts in order to put together a clear picture of what is happening. \nOne important consideration here is the time factor:it is a recognised andrecognisable fact that older people tend to be slower and that they needmore cues before they feel able to take action or make a decision. The problemthen becomes one of balancing the elderly person's need for time with the morepressing physical needs for care, warmth, food,or support which are likely toexist in the situation. The necessary skill is to be able to create a less pressuredsituation in which problems can be seen realistically. \nTo identify the sources of pressure is therefore a priority. The great majorityof people applying for admission to old people's homes do so because of aninability to care for themselves,or an anticipated inability to care for themselvesin their present environment. The underlying factors are predominantly relatedto the standard of accommodation and to the state of health.Problems arisebecause of a cluster of circumstances—an accumulation of difficulties whichgradually arise over a number of years. In addition to this background ofpredisposing factors many admissions to residential care for the elderly seem tobe related to precipitating,short-term factors, such as the illness of a daughter,or changes in family circumstances. It is often possible to provide substitutesupports—home helps, neighbours, etc—on a short-term basis to create thenecessary breathing space. The elderly person can be given space and time torelax for long enough to think more clearly about the alternative courses ofaction. \nIn collecting information the social worker should be able to draw on manydifferent sources,as a member of the agency team and the team of healthprofessionals in the community. It is very important to recognise that illness inthe elderly can show itself in atypical ways.Sudden behaviour changes, confusion, and physical changes, for example, should be investigated by thedoctor as well as the social worker. This may sometimes create difficulties if thedoctor is not currently in touch with the old person or if the patient is unwillingto undergo a medical examination. It is therefore vital for close liaison betweensocial worker, health visitors and doctors to be established to facilitatecommunication in difficult situations. \nThe home help is another important contact between the social worker and theelderly person. She can act as an early warning system for the social worker inreporting difficulties before they reach the crisis stage. As well as this she canalso help to build up the picture of the development of difficulties and give anindication of their cause.Similarly district nurses, volunteers, and othercommunity workers are an important part of the team approach t","cbCainan4JXvMZ9g","https://ap.wps.com/l/cbCainan4JXvMZ9g","pdf",1917401,"English","# Reasons for admission\n## The elderly
