This case report describes a 64-year-old man with a painless right-breast nodule located outside the usual retroareolar region and without gynecomastia. Pathology confirmed pure mucinous carcinoma, with strong estrogen/progesterone receptor expression, low Ki-67, and negative axillary nodes. Management included modified radical mastectomy, axillary dissection, and tamoxifen therapy. The report highlights diagnostic challenges and increasing axillary metastasis incidence in chest mucinous tumors.","| W | J | C | C | World Journal of\u003Cbr>Clinical Cases |\n| --- | --- | --- | --- | --- |\n\nSubmit a Manuscript: [https://www.f6publishing.com](https://www.f6publishing.com) World J Clin Cases 2023 September 6; 11(25): 5954-5961 \nDOI: 10.12998/wjcc.v11.i25.5954 ISSN 2307-8960 (online) \nCASE REPORT \nNon-retroareolar male mucinous breast cancer without gynecomastia development in an elderly man: A case report \nQiang Sun, Xu-Yan Liu, Qi Zhang, Hai Jiang \nSpecialty type: Medicine, research and experimental \nProvenance and peer review: \nUnsolicited article; Externally peer reviewed. \nPeer-review model: Single blind \nPeer-review report’s scientific quality classification \nGrade A (Excellent): 0 \nGrade B (Very good): B, B \nGrade C (Good): 0 \nGrade D (Fair): 0 \nGrade E (Poor): 0 \nP-Reviewer: Kang SY, South Korea; Wu K, United States \nReceived: May 5, 2023 \nPeer-review started: May 5, 2023 \nFirst decision: June 12, 2023 \nRevised: June 30, 2023 \nAccepted: August 9, 2023 \nArticle in press: August 9, 2023 \nPublished online: September 6, 2023 \nQiang Sun, Qi Zhang, Hai Jiang, Department of Breast Surgery, General Hospital of Benxi Iron & Steel Industry Group of Liaoning Health Industry Group, Benxi 117000, Liaoning Province, China \nXu-Yan Liu, Department of Pathology, General Hospital of Benxi Iron & Steel Industry Group of Liaoning Health Industry Group, Benxi 117000, Liaoning Province, China \nCorresponding author: Qiang Sun, PhD, Chief Doctor, Department of Breast Surgery, General Hospital of Benxi Iron & Steel Industry Group of Liaoning Health Industry Group, No. 29 Renming Road, Benxi 117000, Liaoning Province, [China.](China. sq1338@hotmail.com)[ sq1338@hotmail.com](China. sq1338@hotmail.com) \nAbstract \nBACKGROUND \nMale breast cancer (MBC) is an extremely rare condition and accounts for less than 1% of all breast cancers, and malignant tumors occur in less than 1% of the affected men. Mucinous breast cancer is extremely rare and accounts for 2% of all invasive breast cancers. Generally, MBC is accompanied by a retroareolar mass. \nCASE SUMMARY \nHerein, we report a case of male mucinous breast carcinoma (MMBC) without gynecomastia development and with mass localization outside the common retroareolar region, wherein the mass was a painless nodule in the right breast of a 64-year-old man. We also discuss the clinical and pathological characteristics of this unusual tumor. The excised breast specimen showed pure mucinous carcinoma. The patient had strong expression of estrogen and progesterone receptors, a low Ki-67 proliferation index of the tumor cells, and negative pathological axillary lymph nodes. The patient underwent modified radical mastectomy and axillary lymph node dissection, followed by tamoxifen hormone therapy. \nCONCLUSION \nTo the best of our knowledge, this is the first case report of MMBC in the nonretroareolar region of the nipple without gynecomastia development. Mucinous tumors are easily missed during diagnosis, and the incidence of axillary lymph node metastases of chest mucinous tumors has increased. \nKey Words: Breast cancer; Male; Mucinous adenocarcinoma; Nipple; Case report \n©The Author(s) 2023. Non-retroareolar male mucinous breast cancer \nCore Tip: We not only report a rare case, but also discuss the clinical and pathological characteristics of this unusual tumor. Though a summary of a limited number of cases and a comparison with female breast cancer, we reveal some interesting features of male mucinous breast cancer. It is helpful to distinguish the rare male disease. \nCitation: Sun Q, Liu XY, Zhang Q, Jiang H.
