only a dec-ade ago. \nSeveral states are also experi-menting with new systems in an at-tempt to identify the mentally illand separate them for special care. \nThe variety of methods in userange from Oregon's system thathas identified 17 percent of enteringoffenders as acutely mentally ill toNew York's program that attemptsto bring statewide uniformity tomental health assessments in cityand county jails. \nThe Oregon Department of Cor-rections uses a pen-and-pencil-screening instrument on all offend-ers at admission. The system, whichinitially involves only staff,identi-fies that 30 percent of new inmateshave a mental disorder. \nMental health professionalsthen observe the smaller universeand find that slightly more than halfsuffer from acute mental illness and, usually,a co-occurring disabilitysuch as alcohol or drug addiction. \nThe prison can then select in-mates for specialized medical carethat would not be available to thegeneral prison population. \nOregon officials said the initialscreening relies on a simple scoringmethod that is applied uniformly toall entering inmates to assure sys-tem-wide equity.Essentially, thesystem is only to raise an alert thatthe inmate needs professional as-sessment. \nOregon's screening method hasa specific list of symptoms for staffto look for. These include depres-sion,anxiety,suicidal tendenciesand any histories of substance usetrauma, prescription medication orpsychiatric hospitalization. \nSubstance abuse greatly influ-ences symptoms of mental illness. For this reason, and because the ma-jority of inmates with mental illnesshave a co-occurring substance abusedisorder, staff screen for substanceabuse in tandem with mental health. \nSubsequent assessments allowprofessionals to observe the indi-vidual in a drug-free state over timeto separate the causes and effects ofsubstance abuse on menta health. \nIn New York, the State Officeof Mental Health has set up a sys-tem to encourage uniformity ofmental health screening, assessmentand referral procedures in city andcounty jails. \nThe agency began the program \nin 1985 primarily as a suicide screening protocol.Since inception, the agency has made improvementsbased on case studies and expandedthe scope to include a broad rangeof mental health assessments. \nThe New York State Commis-sion of Correction, which accreditsand oversees the development ofnew technology for jails and prisonsin the state, adopted the protocoland now requires all county jailsand penitentiaries and state prisonsto employ it. \nLater,the New York State Of-fice of Alcoholism and SubstanceAbuse Services became involved inthe program, using screening meth-ods developed by the Nathan KlineInstitute for Psychiatric Researchfor use in community settings
