The patient had different implant status bilaterally and was initially medically evaluated to compare simultaneous open reduction and internal fixation versus revision strategies. Consequently, the occurrence of periprosthetic fractures also escalates ([https://pubmed.ncbi.nlm.nih.gov/17477925/](https://pubmed.ncbi.nlm.nih.gov/17477925/)). Periprosthetic fractures represent a notable complication following total hip arthroplasty, frequently affecting elderly patients with multiple comorbidities who are considered high-risk surgical candidates ([https://pmc.ncbi.nlm.nih.gov/articles/PMC10219206/](https://pmc.ncbi.nlm.nih.gov/articles/PMC10219206/)). Given the patient’s characteristics, mainly age and comorbidities, prevention of secondary fall prevention is of paramount importance. Upon occurrence, a decision-making process must be undertaken, considering the patient’s medical condition and pre-injury mobility for optimal rehabilitation ([https://pmc.ncbi.nlm.nih.gov/articles/](https://pmc.ncbi.nlm.nih.gov/articles/)[ ](https://pmc.ncbi.nlm.nih.gov/articles/)[PMC10064640/](PMC10064640/. On the right side there was a cemented femoral stem as opposed to the left side (Fig. 1). The patient was mobilizing independently. \nThe patient was evaluated for her medical status. Considerations regarding the treatment management included simultaneously open reduction and internal fixation of the fractures versus simultaneously total hip revision surgery versus staged revision.
