We ask these to everyone, even though they may not always seem relevant. The purpose of these questions is help us understand more about you and your pregnancy so that we can provide better care.  \nQuestions 1-5  \n1. In the month that you became pregnant, would you say:  \na. You were not using contraception (score = 2)  \nb. You were using contraception, but not on every occasion (like you sometimes used condoms or you missed some pills) (score = 1)  \nc. You always used contraception, but knew that the method had failed at least once (like the condom broke) (score = 1)  \nd. You always used contraception (score = 0)  \n2. In terms of becoming a mother (for the first time or again), do you feel that your pregnancy happened at the:  \na. Right time (score = 2)  \nb. OK, but not quite right time (score = 1)  \nc. Wrong time (score = 0)  \n3. Just BEFORE you became pregnant would you say:  \na. You intended to get pregnant (score = 2)  \nb. Your intentions kept changing (score = 1)  \nc.
