A prospective randomized single-center superiority trial compared high-flow nasal cannula (HFNC) with NIV in adult ED patients with ACPE, assessing improvement in respiratory rate and multiple clinical outcomes. Among 178 randomized participants, the change in respiratory rate at 120 minutes was similar in intention-to-treat and per-protocol analyses, with no meaningful differences in vital signs, arterial blood gas measures, or dyspnea scores.
