Colitisassociated colorectal cancer (CA-CRC) refers to the increased risk of colorectal cancer in individuals with chronic inflammatory bowel diseases (IBD) such as ulcerative colitis and Crohn’s disease. \nMethods This narrative review examines the link between chronic inflammation and CA-CRC. A comprehensive literature search was conducted using PubMed, Scopus, and Web of Science, focusing on studies published between 2000 and 2024. Studies were selected based on relevance to the role of inflammation in CA-CRC, specifically targeting molecular pathways and clinical implications. Both clinical and mechanistic studies were reviewed. \nConclusion Sustained inflammation in the colon fosters a pro-tumorigenic environment, leading to the initiation and progression of CA-CRC. Prevention strategies must focus on controlling chronic inflammation, optimizing IBD management, and implementing regular screenings. Emerging therapies targeting key inflammatory pathways and immune responses, along with microbiome modulation, hold promise for reducing CA-CRC risk. Understanding these molecular mechanisms provides a path toward personalized treatment and better outcomes for patients with IBD at risk of colorectal cancer. \nKeywords Chronic inflammation · Colitis-associated colorectal cancer · Inflammatory bowel diseases · Pro-inflammatory mediators \nIntroduction \nThe well-established link between chronic inflammation and cancer development is particularly evident in colorectal cancer (CRC), which ranks as the second leading cause of cancer-related deaths in Western countries [1, 2] . Colitis-associated colorectal cancer (CA-CRC) is a significant \ncontributor to morbidity and mortality among patients with inflammatory bowel disease (IBD), such as ulcerative colitis (UC) and Crohn’s disease (CD) [3] . Although only 1–2% ofIBD patients develop CRC, CA-CRC accounts for approximately 15% of all IBD-related deaths [4] . As the global incidence of IBD continues to rise, there is an urgent \n* Behzad Baradaran [behzad_im@yahoo.com](behzad_im@yahoo.com) \nVahid Khaze Shahgoli \n[shahgoli@health.sdu.dk](shahgoli@health.sdu.dk) \nSaeed Noorolyai \n[saeednoorolyai@yahoo.com](saeednoorolyai@yahoo.com) \nMahya Ahmadpour Youshanlui \n[mahyaahmadpouryoushanlui@gmail.com](mahyaahmadpouryoushanlui@gmail.com)[ ](mahyaahmadpouryoushanlui@gmail.com)Hossein Saeidi \n[saeedi.hossein1995@gmail.com](saeedi.hossein1995@gmail.com) \nHadi Nasiri \n[hadinasiri709@gmail.com](hadinasiri709@gmail.com) \nBehzad Mansoori \n[b.mansoori_lab@yahoo.com](b.mansoori_lab@yahoo.com) \nUffe Holmskov \n[uholmskov@health.sdu.dk](uholmskov@health.sdu.dk) \n1 Faculty of Medicine, Immunology Research Center, Tabriz University of Medical Sciences, Tabriz, Iran \n2 Department of Molecular Medicine, University of Southern Denmark, Odense, Denmark \n3 Molecular and Cellular Oncogenesis Program, The Wistar Institute, Philadelphia, PA, USA \nneed to better understand the mechanisms that link chronic inflammation to colorectal cancer development [5] . \nChronic inflammation is recognized as a key driver in the progression of many diseases, including cancer. Within the colon, persistent inflammation plays a crucial role in the onset of CA-CRC. While patients with IBD can develop sporadic neoplasia, chronic inflammation remains the primary factor contributing to cancer risk [6] .
