only independent news service for the corrections professlonal \n# U.S.Sees Targeted Tests As EffectiveTo Attack Hepatitis C At Lowest Cost\n\nThe Federal Bureau of Prisons has identified targeted testing of inmates with known risk factors forhepatitis C as the most effectiveway to attack the disease at the low- est cost \n\"There are now good datashowing if you do targeted screen-ing and take a good history, you canidentify the vast majority ofinmateswith HCV,\"said Dr NewtonKendig.medical director for the bu-reau's heaith services division. \nThe bureau, like many otherprison systems, conducts a risk fac-tor assessment on inmates whocome into the system to determinewhether an inmate is at risk forHCV infection. \nThe most important question toask,of course,is whether an inmatehas ever injected drugs;unlikehepatitis B or HIV,HCV can be ac-quired very rapidly with just one in-cident of injection drug use. \n\"Clearly,it is the injection drugusing inmates who are at highestrisk in our settings,\"noted Kendig. \nThe federal prison system hasdiagnosed 11,000 of its 160,000inmates with HCV.Unlike someprison systems, the bureau has a di-verse population, different types ofinmates with very different risk fac- \ntors.That's why it makes sense toconduct targeted testing as opposedto universal testing, said Kendig. \nBut in some correctional sys- tems,it might be appropriate to ap-ply universal HCV testing, the doc-tor said. \n“Universal testing may be ap-propriate in other systems wherethere are very high rates of injecticndrug use in a certan locale,\"hesaid\"That decision needs to bemade on a system by sysiem basis.\" \nEducation is a major componenfof the federal approach to HCV. \nOfficials present HCV informa-tion and education to inmates com-ing into the system, explaining howthe virus is spread and how to avoidinfection.Officials also tell inmatesthat alcohol use greatly hastensHCV progression. \nIt is important,Kendig said, tolook at the length of incarcerationbefore deciding on a course of ac-tion. \n\"We have jails where inmatesare only with us for days and inthose cases the focus really is oneducation,it is not on treatment,\"Kendig said. \nIn most instances, the bureauwill only treat inmates with 12 months or more left on their seni- \nPage 2 \ntences so that they can complete thetreatments while incarcerated. \n“We are reluctant to interrupttherapy if we can avoid that,\"Kendig said. \n“That is why we are less likelyto start someone on therapy who ison their way out into the commu-nity.Instead,we will do an evalua-tion and then refer them to a physi-cian in the community. But we havetreated a few people who have lessthan 12 months left so it is a factorwe look at.\" \nKendig stressed that HCVtreatment decisions must be deter-mined on a case-by-case basis. \nThere are no hard and fast rules,he said.Nevertheless, the Bureau ofPrisons tends to follow certainguidelines when making treatmentdecisions. \nThe bureau, for example,re-quires liver biopsies on most in-mates who are candidates for treat-ment to determine disease statusand likely progression. In mostcases, the bureau will only treat in-mates with at least some fibrosis orscarring of the liver. \n“We don't have a strict cut off,”K","cbCaimtAukEBj19Z","https://ap.wps.com/l/cbCaimtAukEBj19Z","pdf",2419116,"English","# Correction Digest\n## U.S. Sees Targeted Tests As Effective To Attack Hepatitis C At Lowest Cost\n##
