Rituximab plus chemoimmunotherapy (R-CHOP) improves outcomes, yet diffuse large B-cell lymphoma (DLBCL) still faces relapse and early resistance, affecting roughly 30–40% of patients. A narrative review \nAnna Aureli 1, Giulia Lanzilli 1, Stefano Vito Boccadamo Pompili 1, Giulio Cesare Spagnoli 1, Giuseppe Sconocchia 1,2* \nand Sara Caratelli 1 \n1 Department of Biomedicine, Institute of Translational Pharmacology, National Research Council (CNR), Rome, Italy, 2Saint Camillus, International Medical University (UNICAMILLUS), Rome, Italy \nB-cell lymphomas (BCLs) are a heterogeneous group of blood cancers whose treatment has been significantly transformed by immunotherapy. The use of the anti-CD20 monoclonal antibody rituximab, combined with chemotherapy (RCHOP), has markedly improved patient outcomes. However, some subtypes, particularly Diffuse Large B-Cell Lymphoma (DLBCL), remain challenging to treat due to frequent relapses and/or early resistance to therapy. Standard frontline immunochemotherapy is successful in about 60% of DLBCL cases, whereas 30- 40% experience relapse or refractory disease, thus highlighting the urgent need for more effective therapeutic strategies. More recently, advanced immunotherapies have been investigated, relying on the use of innovative bi-and trispecific antibodies, antibody-drug conjugates, and immune checkpoint inhibitors . Furthermore, immune effector cell-based therapies, such as CAR-T and CARNK cells, have been developed. Among these, CAR-T cell therapy targeting CD19 achieved high response rates in patients with relapsed or refractory DLBCL, emerging as new pivotal therapeutic option. In this narrative review, we provide an overview of current immunotherapy treatments for DLBCL, including underlying mechanisms, clinical outcomes, and safety profiles. \nKEYWORDS \nB-cell lymphoma, bispecific antibody, CAR-NK, CAR-T, checkpoint inhibitors, FcgRs, immunotherapy, monoclonal antibody \nAbbreviations: ADCC, antibody-dependent cellular cytotoxicity; ADCP, antibody-dependent cellular phagocytosis; ADCs, antibody–drug conjugates; ASCT, autologous stem cell transplantation; BCL, B-cell lymphomas; BLCL, B-lymphoblastoid cell line; BsAbs, bispecific antibodies; CAR, chimeric antigen receptor; CDC, complement-dependent cytotoxicity; CPI, immune checkpoint inhibitors; CR, complete remission; CRS, cytokine release syndrome; DLBCL, diffuse large B-cell lymphoma; EMA, European Medicines Agency; FcgCR, Fc gamma chimeric receptors; FcgR, Fc gamma receptors; FDA, Food and Drug Administration; FL, follicular lymphoma; GvHD, graft-versus-host disease; ICANS, immune effector cell–associated neurotoxicity \nFrontiers in Immunology 01 [frontiersin.org](frontiersin.org) \n1 Introduction \nLymphomas are a","cbCaihMuyA8OsApj","https://ap.wps.com/l/cbCaihMuyA8OsApj","pdf",3058643,16,"English","# Introduction\n## Classification and epidemiology of lymphomas\n## Subtypes and prognostic relevance in DLBCL\n## Molecular features (e.g., MYC) and targeted implications","[{\"question\":\"Why is immunotherapy important for diffuse large B-cell lymphoma (DLBCL)?\",\"answer\":\"Immunotherapy has transformed treatment outcomes, but DLBCL remains challenging because a substantial fraction of patients relapse or develop early resistance to standard chemoimmunotherapy.\"},{\"question\":\"How does R-CHOP affect outcomes in DLBCL?\",\"answer\":\"R-CHOP improves patient outcomes in the front-line setting, but approximately 30–40% still experience relapse or refractory disease despite treatment.\"},{\"question\":\"What therapeutic approaches are highlighted beyond monoclonal antibodies?\",\"answer\":\"The review covers advanced strategies including bispecific/trispecific antibodies, antibody-drug conjugates, immune checkpoint inhibitors, and immune effector cell therapies such as CAR-T and CAR-NK.\"}]","Immunotherapy of diffuse large B-cell lymphoma: from monoclonal antibodies to cellular therapies - A narrative review | PDF",1790121285,40,{"code":4,"msg":82,"data":100},[101,105,109,113,118,123,125,130,135,138,142],{"id":22,"doc_module":4,"doc_module_name":25,"category_name":102,"show_sort_weight":103,"slug":104},"Story & Novel",90,"story-novel",{"id":26,"doc_module":4,"doc_module_name":25,"category_name":106,"show_sort_weight":107,"slug":108},"Literature",80,"literature",{"id":33,"doc_module":4,"doc_module_name":25,"category_name":110,"show_sort_weight":111,"slug":112},"Exam",70,"exam",{"id":114,"doc_module":4,"doc_module_name":25,"category_name":115,"show_sort_weight":116,"slug":117},5,"Comic",60,"comic",{"id":119,"doc_module":4,"doc_module_name":25,"category_name":120,"show_sort_weight":121,"slug":122},6,"Technology",50,"technology",{"id":86,"doc_module":4,"doc_module_name":25,"category_name":29,"show_sort_weight":98,"slug":124},"healthcare",{"id":126,"doc_module":4,"doc_module_name":25,"category_name":127,"show_sort_weight":128,"slug":129},8,"Research & Report",30,"research-report",{"id":131,"doc_module":4,"doc_module_name":25,"category_name":132,"show_sort_weight":133,"slug":134},9,"Religion & Spirituality",20,"religion-spirituality",{"id":133,"doc_module":4,"doc_module_name":25,"category_name":136,"show_sort_weight":133,"slug":137},"World Cup","world-cup",{"id":139,"doc_module":4,"doc_module_name":25,"category_name":140,"show_sort_weight":139,"slug":141},10,"Lifestyle","lifestyle",{"id":143,"doc_module":4,"doc_module_name":25,"category_name":144,"show_sort_weight":114,"slug":145},19,"General","general"]
