However, certain atypical cases exist in which malignancies do not exhibit restricted water molecule diffusion. \nCase Presentation: \nWe present the case of a 66-year-old male with moderately differentiated rectal adenocarcinoma showing heterogeneous diffusion restriction on MRI. The tumor surface demonstrated diffusion restriction (hyperintense on DWI, hypointense on ADC), whereas the base did not. Quantitative ADC analysis showed mean values of 0.82 × 10-3 mm2/s at the surface and 1.36 × 10-3 mm2/s at the base. \nConclusion: \nThis case highlights that regions with lower cellularity in moderately differentiated adenocarcinoma may lack diffusion restriction. Correlating imaging findings with histopathological results is essential to prevent diagnostic misinterpretation. \nKeywords: Diffusion-weighted imaging (DWI), Apparent diffusion coefficient (ADC), Colorectal cancer, Rectal adenocarcinoma, Cell density, Diffusion-restriction. \n\n| Article History | Received: July 08, 2025 | Revised: October 25, 2025 | Accepted: November 10, 2025 |\n| --- | --- | --- | --- |\n\n1. INTRODUCTION \nAccording to Globocan 2022, colorectal cancer ranks among the most prevalent cancers worldwide, placing within the top three for both incidence and mortality-following lung cancer overall, breast cancer in women, and prostate cancer in men [1] . Histopathologically, colorectal cancer comprises various types, with adenocarcinoma representing the vast majority of all cases [2] . Common clinical manifestations include hematochezia, bowel habit changes, abdominal pain, and unintended weight loss. Diagnosis primarily relies on clinical presentation, endoscopic evaluation, and histopathological confirmation through biopsy. Genetic testing may also be indicated when hereditary predisposition is suspected. Multiparametric MRI, including DiffusionWeighted Imaging (DWI), Apparent Diffusion Coefficient (ADC) mapping, and contrast-enhanced sequences are valuable noninvasive tools for assessing malignancy in general, and colorectal tumors in particular [5] .
