Here, we performed a literature review using articles listed in electronic databases, such as PubMed, Scopus, MEDLINE, and Google Scholar. In this review article, we describe contradictory data regarding the role of folic acid in cancer development and progression. While some studies have confirmed its beneficial effects in diminishing the risk of various cancers, others have reported a potential carcinogenic effect. The current narrative review elucidates these conflicting data by highlighting the possible molecular mechanisms explaining each point of view. Further multicenter molecular and genetic studies, in addition to human randomized clinical trials, are necessary to provide a more comprehensive understanding of the relationship between folic acid and cancer. \nKeywords \nBreast cancer, carcinogenic, colorectal cancer, folate, folic acid, lung cancer, prostate cancer \nDate received: 14 October 2023; accepted: 11 December 2023 \n1 Department of Pharmacology, Faculty of Medicine, Assiut University, Assiut, Egypt \n2Department of Basic Medical Sciences, Faculty of Medicine, Aqaba Medical Sciences University, Aqaba, Jordan \n3Department of Basic Medical Sciences, Faculty of Medicine, Yarmouk University, Irbid, Jordan \n4Intern Doctor at Princess Basma Hospital, Ministry of Health, Irbid, Jordan \nCorresponding author: \nRomany Helmy Thabet, Department of Basic Medical Sciences, Faculty of Medicine, Aqaba Medical Sciences University, C12 Al Haddab, Parallel Road, South Beach, Aqaba 77110, Jordan. \nEmail: [romanyh.gerges@amsu.edu.jo](romanyh.gerges@amsu.edu.jo) \nCreative Commons Non Commercial CC BY-NC: This article is distributed under the terms of the Creative \nCommons Attribution-NonCommercial 4.0 License ([https://creativecommons.org/licenses/by-nc/4.0/](https://creativecommons.org/licenses/by-nc/4.0/)) which permits non-commercial use, reproduction and distribution of the work without further permission provided the original work is attributed as specified on the SAGE and Open Access pages ([https://us.sagepub.com/en-us/nam/open-access-at-sage](https://us.sagepub.com/en-us/nam/open-access-at-sage)). \nIntroduction \nFolate is the natural form of vitamin B9 . It is water-soluble and naturally found in plants and vegetables, such as dark leafy greens, citrus fruits, and meat products. Folate is also added to foods and sold asa supplement in the form of folic acid. Infact, this form is better absorbed than the folate form from food sources, 85% vs. 50%, respectively. This essential nutrient is a crucial part of many diverse biological processes. For example, it has an indispensable role in cell maturation and nucleotide synthesis through the methylation of uracil to thymine, which is essential for DNA synthesis and repair. 1,2 Moreover, folate metabolism is fundamental to both normal and cancer cell proliferation. Previous studies have suggested that folic acid has homocysteine lowering effects and is possibly beneficial for the prevention of cardiovascular disease (CVD) and stroke.3,4 On the contrary, folic acid might also promote the progression of atherosclerosis because of its stimulatory effect on cell proliferation.
