Recent studies indicate that having a billed AWV is associated with increases in preventive care services such as vaccinations, cancer screening, and dementia diagnosis. However, there is variation in patient subpopulations who receive the AWV and the opportunities to enhance implementation of the visit to better meet the needs of older adults. This session offers insights into the AWV from different contexts, including population-, health system-, and implementation-level. The first session uses a nationally representative sample of Medicare beneficiaries to provide an overview of the differential uptake of theAWV across Medicare insurance coverage type (Traditional Medicare vs. Medicare Advantage), highlighting heterogenous impact by age, race, and health status. The next session examines theAWV at a health system level, by drawing on electronic health record data to identify the frequency of follow-up actions taken after memory concerns are reported during theAWV.The third session presents a pilot study seeking to improve dementia care pathways by testing workflow enhancements via nurse-led AWVs. \nAttendees of this symposium will take away key insights for potential","cbCaijAhXmAcQDkQ","https://ap.wps.com/l/cbCaijAhXmAcQDkQ","pdf",322343,"English","# Symposium overview\n## AWV purpose and patient-reported prevention planning\n## Evidence linking AWV billing to preventive services\n## Population-level uptake differences\n## Health system-level follow-up actions after memory concerns\n## Nurse-led workflow enhancements for dementia care pathways","[{\"question\":\"What is the Medicare Annual Wellness Visit (AWV) designed to do?\",\"answer\":\"The AWV collects patient-reported information and supports creating a personalized prevention plan to address identified patient needs.\"},{\"question\":\"How does receiving a billed AWV relate to preventive care services?\",\"answer\":\"Recent studies indicate that having a billed AWV is associated with increases in preventive care services such as vaccinations, cancer screening, and dementia diagnosis.\"},{\"question\":\"What approaches does the symposium use to enhance AWV implementation for older adults?\",\"answer\":\"It examines differential uptake across coverage types, evaluates follow-up actions after memory concerns using electronic health record data, and tests nurse-led workflow enhancements to improve dementia care pathways.\"}]","ENHANCING CARE FOR OLDER ADULTS THROUGH THE MEDICARE ANNUAL WELLNESS VISIT - Symposium - Session 25 | PDF",1790635545]
