Inclusion criteria included locally advanced or metastatic gallbladder cancer deemed unresectable by multidisciplinary team (MDT) consensus; age 18–75 years; ECOG performance status ≤2; and AG as first-line chemotherapy. Treatment response was assessed using RECIST 1.1 criteria. Primary endpoints were overall response rate (ORR) and progression-free survival (PFS); secondary endpoints included disease control rate (DCR), overall survival (OS), and adverse events incidence. \nResults: Thirty-one patients were enrolled. The ORR was 48.4%, and DCR was 90.3% . The median PFS was 8.4 months (95% CI, 5.0–11.8), and the median OS was 20.9 months (95% CI, 16.8–24.9). Among the 11 patients with locally advanced disease, 7 patients (63.6%) successfully converted to a resectable state and underwent surgery. Grade 3/4 treatment-related adverse events occurred in 11 patients (35.5%), predominantly hematologic toxicities, and no treatment-related deaths or treatment discontinuations due to adverse events were observed. \nConclusion: First-line AG chemotherapy demonstrated promising efficacy and manageable safety for patients with unresectable GBC, with a substantial portion achieving disease control and some converting to resectable status. \n1 | Introduction \nGallbladder cancer (GBC) is a rare but highly aggressive malignancy with a poor prognosis. The incidence of GBC varies geographically, with an estimated annual rate of approximately 10 cases per 100,000 population [1, 2] . Early diagnosis of GBC is challenging as it is initially asymptomatic, leading to advanced and terminal disease at the time of diagnosis [3] . Consequently, the survival rate of patients with GBC remains low, with less \nthan 5% of patients with advanced disease surviving at five years [4]. Survival is particularly low in patients with unresectable tumors, with a median survival time of only about six months. \nAlthough GBC is usually classified as a subtype of biliary tract cancer (BTC), it is increasingly recognized as a distinct entity with unique molecular biological mechanisms and clinical characteristics, necessitating dedicated exploration of its treatment options. Due to its limited incidence, most clinical studies \nThis is an open access article under the terms of the Creative Commons Attribution-NonCommercial-NoDerivs License, which permits use and distribution in any medium, provided the original work is properly cited, the use is non-commercial ","cbCaieiXA3iEnYJq","https://ap.wps.com/l/cbCaieiXA3iEnYJq","pdf",1986696,"English","# Abstract\n## Purpose\n## Materials and Methods\n## Results\n## Conclusion\n# Introduction","[{\"question\":\"What is the main purpose of this study?\",\"answer\":\"To evaluate the efficacy and safety of nab-paclitaxel combined with gemcitabine as first-line chemotherapy for unresectable gallbladder cancer.\"},{\"question\":\"How were efficacy outcomes measured?\",\"answer\":\"Treatment response was assessed using RECIST 1.1, with overall response rate (ORR) and progression-free survival (PFS) as primary endpoints.\"},{\"question\":\"What were the key efficacy and safety results?\",\"answer\":\"Among 31 patients, ORR was 48.4% and DCR was 90.3%, with median PFS 8.4 months and median OS 20.9 months.
