physicians now prefer to call it,epinephrine.Per-haps the most striking effect of this release of epinephrine is the risein blood pressure, due in part to the constriction of certain smallarteries. The result is a decrease in the blood supply to internalorgans whose immediate response is not required by the “dangerous”situation. At the same time, there is a dilation of arteries whichsupply the muscles of the skeleton, providing them more richly withblood so that the animal may fight more effectively,or run away atgreater speed.Epinephrine also acts directly on the heart muscle, causing it to beat faster and more forcefully. \nNor does the body depend on the adrenals alone to liberateepinephrine.Indeed, this chemical which so rallies the body's forces \n工 \nfor struggle or flight,or substances very much like it,are also re-leased at the ends of sympathetic nerve fibers of the autonomicnervous system. Some of these nerve fibers end on muscle cells inthe walls of blood vessels;others end on heart muscle cells. Thus, the potent chemicals are released at the very site of their ultimateaction, helping to prepare the body for the dangers at hand. \nWhile this “fight or filight”mechanism can be essential to thesurvival of animals in the wild,it is often of no value to civilizedman-and may well be detrimental at times.Confronted with situa-tions which provoke fear or rage, modern man often finds that hemay neither fight nor fiee;his culture denies him either choice.Con-sequently,the elevated blood pressure, and the rapid, powerfulbeating of the heart serve no purpose whatever, and in some persons(notably those suffering from cardiovascular disease)this responsemay even precipitate a heart attack or stroke. \nMedicine has therefore long been concerned with drugs able toblock the action of epinephrine and similar substances which arereleased by the sympathetic nerve fibers and which act on bothsmooth and cardiac muscle.Such drugs are known as adrenergicblocking agents, and fall into two groups. \nThe older drugs, now categorized as alpha-adrenergic blockingagents, neutralize the constricting effect of epinephrine on blood ves-sels,and this helps keep blood pressure down. The newer drugs, known as beta-adrenergic blocking agents, neutralize the effects ofepinephrine on heart muscle, and counteract other effects.Neithertype of blocking agent actually destroys the epinephrine or similarsubstances.Instead,it seems that the molecules of these chemicalsubstances combine with the muscle cells at certain specific sites, effectively blocking the union of epinephrine with the cell—a unionwhich must take place before epinephrine can affect the cell. \nDr.Neil C.Moran(Emory University,Atlanta)told an Acad-emy Conference on New Adrenergic Blocking Drugs how Powelland Slater of Lilly Research Laboratories in 1957 discovered that achemical called dichloroisoproterenol(DCI)was antagonistic toepinephrine in a way that at that time was unique. In 1958,Drs.Moran and Perkins uncovered the adrenergic blocking effect of thischemical on the heart.Dr.Moran suggested then that DCI be classi-fied as a beta-adrenergic blocking agent. The general structuralspecifications for beta-adrenergic blocking agents were rapidly es-tablished.Since that time, other drugs with beta-adrenergic proper-ties have been synthesized and studied. \nThe structural similarity of the beta-adrenergic blocking agents \nto epinephrine and chemicals like epinephrine is amazing, despitetheir apparent differences in biological activity. But there are crucialdifferences in chemical structure. It almost seems as though,to putit crudely,a part ","cbCaijIAHNz01WOl","https://ap.wps.com/l/cbCaijIAHNz01WOl","pdf",1519409,32,"English","# DRUGS THAT BLOCK
