It occurs in all ages, races, and genders and can stem from obstetric causes such as acute fatty liver of pregnancy, amniotic fluid embolism (rich in tissue factor), abruptio placenta, postpartum hemorrhage (PP), HELLP syndrome, intrauterine fetal demise (IUFD), and septic abortion, as well as TF from the placenta. Non-obstetric causes include solid tumors, large aortic aneurysms, and continuous or intermittent exposure to small amounts of tissue factor. Chronic DIC can be compensated by the liver and bone marrow, leading to just thrombosis and venous thromboembolism, with little obvious clinical signs or laboratory findings of DIC. Acute DIC, however, presents with hemorrhage and consumptive coagulopathy, where the liver and bone marrow cannot compensate for the consumption of clotting factors and platelets. Lab findings in acute DIC include low platelets, prolonged prothrombin time (PT) and activated partial thromboplastin time (aPTT), and high D-dimer. Hemorrhage in DIC can be life-threatening, manifesting as petechiae on the soft palate, trunk, and extremities, or ecchymosis at trauma sites and venipuncture sites. It may also be accompanied by accelerated fibrinolysis in some cases. Treatment of DIC involves addressing the underlying cause, monitoring vital signs, and assessing the extent of hemorrhage and thrombosis. Management also includes correcting hypovolemia and, in severe cases, supporting organ function. Specific treatments might involve platelet transfusion, cryoprecipitate, and packed red blood cells. Heparin may be used in cases of chronic DIC or if thrombosis predominates, but it should be used cautiously in acute bleeding.
