Advances in our understanding of complex pathophysiologic mechanisms have opened new therapeutic avenues, underscoring the need for a comprehensive diagnostic approach.  \nESSENTIAL POINTS: Nearly one-half of polyneuropathy cases remain idiopathic. Diabetes is the most common risk factor for peripheral neuropathy, present in one-third of patients. Improved diagnostic precision and insights into hereditary and acquired causes are crucial for effective management and treatment strategies. Care for these patients should integrate advanced techniques in the diagnosis of neuropathies.  \nINTRODUCTION  \nT  \nhe cornerstone of neuropathy diagnosis remains comprehensive clinical assessment, thorough history taking, and physical examination. These elements provide essential insights into the nature, etiology, and prognosis of the underlying conditions and neuropathy. A detailed patient history can reveal critical  \ninformation about the onset, distribution, and characteristics of symptoms, as well as potential risk factors or exposures. This information is vital for guiding further diagnostic steps and developing appropriate treatment plans. Physical examination allows for objective evaluation of sensory, motor, and autonomic functions, helping to characterize and determine the extent of peripheral nerve involvement. By combining a patient’s subjective experience with objective clinical findings, neurologists can build a more complete picture of polyneuropathy and its possible etiologies (FIGURE 1-1) . Ancillary testing, such as electrophysiology (ie, nerve conduction studies and EMG) and imaging, can complement clinical diagnosis but do not replace the need for a thorough clinical assessment.
