Potential cost savings, reduced hospital overcrowding, and freed up capacity to meet unmet surgical need are systemlevel benefits. More importantly, day surgery improves patient safety and convenience due to reduced time in the hospital and greater attention to pre-operative patient preparation, safe recovery, and post-discharge care.  \n• Conditions for performing day surgery are attainable for middle income health systems. Aseptic [sterile] surgical techniques, fast-acting anesthesia, and minimally invasive surgery techniques are increasingly available in middle income countries to ensure safe surgery with rapid recovery. The necessary protocols for patient selection, preparation, and post-discharge follow-up care can be developed based on experience in other countries, such as India, the Philippines, and Malaysia.  \n• Effective day surgery promotion requires not only authorization and encouragement, but also financial incentives. Countries with similar payments for day and inpatient surgery are more successful at promoting day surgery. Additional measures, such as setting day surgery as the default for simple surgeries and requiring pre-approval for inpatient surgery, provide support for this transition.  \nKEY MESSAGES  \n• Viet Nam could benefit from shifting to day surgery through reform of provider payment and development of regulations and protocols. Minimally invasive surgery is widely available in Viet Nam, but financial incentives, clinical practice norms, and patient expectations inhibit shifting today surgery. Diagnosis-related groups payment reforms can create the incentives that motivate the health sector to develop modern day surgery in Viet Nam.  \nWHAT IS DAY SURGERY?  \nThe term “day surgery”1 is not simply surgery with admission and discharge on the same day, but rather the practice of admitting into a health facility carefully selected and prepared patients for a planned, non-emergency procedure that avoids the need for an overnight stay (Castoro et al. 2007) . Day surgeries are case types that would traditionally have been treated as inpatient cases. However, through careful patient selection/screening and education, dedicated day surgery facilities and teams, and compliance with enhanced recovery after surgery (ERAS) protocols spanning pre-hospital to post-discharge stages, need for an overnight stay can safely be avoided. Although the standard definition of day surgery excludes emergency cases, some types of emergency surgeries can be performed on a same-day basis, such as incision and drainage of an abscess.  \nDay surgery continues to expand as a share of all elective surgeries and accounts for a high share of surgeries in many countries. A high share of two common elective surgeries, cataract surgery and tonsillectomy, are performed as day surgery (Figure 1) in most OECD countries, including in middle income countries like Mexico and Costa Rica. No single factor can explain the considerable variation in uptake of day surgery. Factors perceived to have driven the shift in some countries include advances in medicine, economic pressures, patient safety and preferences, limited hospital bed supply and long wait times, and financial and non-financial measures. Barriers to change include misaligned financial incentives or disjuncted financial responsibilities and fear of lower quality for day surgery compared to inpatient surgery (Kreutzberg et al. 2024) .  \n1 Also known as “ambulatory surgery” or “outpatient surgery.”  \nFIGURE 1.
