When a medical problem is discovered during pregnancy that can endanger the life of the woman and/orthe life of the fetus, the women are faced with a paradoxical decision-whether to terminate the pregnancy proactively or to continue it, despite the risks. The purpose of the current study is to examine the emotional experience of women who have undergone both procedures-in vitro fertilization (IVF) and induced termination of pregnancy (TOP) due to a health problem, as this experience has not received sufficient expression in the literature in recent years. Twelve Israeli women who underwent IVF treatments, became pregnant, then underwent an induced TOP due to a health problem, were interviewed. The research data was collected through semi-structured in-depth interviews and analyzed according to “grounded theory”. Findings indicate that pregnancy loss in the context of IVF treatments is a cyclical, procedural, ongoing emotional and medical experience that involves dealing with loss and grief at each stage. Worldwide, one in six couples encounters fertility challenges at a stage when they desire a child2. This issue has been a prominent health concern in recent years among women around the world and represents a significant physical and psychological stressor3. In-vitro fertilization (IVF) stands as one of the most common, lengthy and complex fertility treatments available4.  \nThe number of women in Israel who undergo fertility treatments is the largest in the world5. This information is not surprising considering Israeli culture and Jewish tradition that emphasize the birth of children as a supreme value5. Hence, when the difficulty of conceiving arises, many women experience feelings of failure and meaninglessness5,6.  \nThe difficulty in conceiving can be due to both a fertility problem in the woman and a fertility problem in the man, though in many cases the reason for the lack of success is not clear7. However, since fertility treatmentsand pregnancy occur exclusively in a woman’s body, many women experience a greater personal responsibility than men in the fertility process8. Many women describe the treatment journey as climbing a mountain, where each step intensifies additional emotional suffering, alongside the growing fears of failure6. Research consistently shows that the most stressful moments during the treatments are the waiting period before the pregnancy test after the fertility treatment and discovering treatment failure6.  \nWhen the fertility treatment succeeds and a pregnancy occurs, women experience profound hope, which is devastatingly shattered if the pregnancy ends, both through a natural miscarriage and/or an induced TOP due to a health problem9, 10. In fact, usually, the first IVF treatment is not successful, and no pregnancy develops. The live birth success rate following IVF treatments has recently been reported in the range of 36–41% overall, in data pooled across programs after adjusting for repeated transfers11.
