professional  \nLiver Biopsies Can Be Cost-Effective InSelecting HCV Prisoners For Treatment  \nLiver biopsies are a cost effective way of evaluating and treating in-mates for hepatitis C(HCV)andcan serve as a critical component ofa comprehensive screening andtreatment strategy.  \nVirginia Commonwealth Univer-sity presented a model to a confer-ence on HCV in correctional set-tings that estimated cost savings ofmore than $100,000 for every 100inmates evaluated and treated.  \nVCU conducted the analysis inconjunction with the Virginia De-partment of Corrections.  \nThe model compared the hypo-thetical cost of evaluation andtreatment of 100 infected inmateswith and without a liver biopsy.  \nVCU predicted the departmentcould save $1,247 for every HCV-positive inmate using a biopsy strat-egy to determine who should andshould not be treated.  \nThe study was based on the fol-lowing two assumptions that thedepartment has to be willing:  \nTo treat those with HCV whomeet pre-determined criteria.  \n■ To not treat inmates who areunlikely to progress to significantliver disease during incarceration.  \n\"This looks at two differentstrategies,\"explained Dr.Richard  \nK.Sterling,a physician with theHepatology Section and LiverTransplant Program at VCU.  \nSterling presented the findingsduring the conference on the man-agement of HCV in prisons.  \n“One is a no-biopsy strategywhere you treat all inmates withoutbiopsy and the other is a biopsystrategy where you only treat in-mates who have clinically signifi-cant disease.\"  \nThe study defined\"significantdisease”conservatively—a histo-logic activity index(HAI)greaterthan four and any fibrosis.  \nInmates with an HAI of less thanfour and no fibrosis were not treat-ment candidates,a cut-off point thateliminated 15 percent of the model's100 inmates from treatment consid-eration,resulting in $124,679 incost savings.  \nThe department could reducecosts further by treating only in-mates with fibrosis and deferringtreatment for those inmates withoutfibrosis,regardless of their HAIscore,a strategy that would elimi-nate 31 percent of the patients fromtreatment.  \nThe cost of treating HCV wouldfall further by treating only inmateswith bridging fibrosis and not portal  \nPage 2  \nfibrosis,a strategy that would onlyrequire the system to treat 24 per-cent of the model's 100 patients.  \nThis means,the model's costsavings are determined by treatmentcut off points.  \nSterling based the study on 100hypothetical patients,80 of whomhad HCV genotype 1,requiringnearly 12 months of treatment while20 percent were genotype non-1,which calls for six months of ther-apy.  \nSterling stressed that a liver bi-opsy is one component of an overallHCV screening and treatment strat-egy and part of a “rational approachfor the management of HCV in theprison population.\"  \nCorrectional systems,he said,\"need to develop a rationale plan forevaluations.Liver biopsies are
