communicable1diseases among the nation's two million inmates mandate a regular regimen of testing and treatment, concludes the National Commissionon Correctional Health Care. \nThe commission said inmatesaccount for nearly 17 percent of thenation's AIDS cases and 19 percentof all HIV-positive individuals. \nHepatitis and other communi-cable illnesses are also dispropor-tionate in the prison population, posing threats for corrections staffand the public when inmates with-out treatment are released. \nCongress in 1997 instructed theJustice Department to determine theimpact of inmate health on the lar-ger population. The National Com-mission on Correctional HealthCare completed the study and pub-lished its results. \n\"Because they have a large andconcentrated population of indi-viduals at high risk for disease, prisons and jails offer a unique op-portunity for improving diseasecontrol in the community by provid-ing comprehensive health care anddisease prevention programs to in-mates,\"the commission report said. \nThere are more than three mil- \nlion persons with hepatitis C infec- \ntion in the United States and nearlyone-third of them have spent time incustody. \nMost offenders spend relativelylittle time in custody and as a resultnearly 12 million are released eachyear, the commission said. \nWithin those released, theremay be as many 38,500 cases ofAIDS and 465,000 cases of sexuallytransmitted diseases of all typesTuberculosis is also vastly over-represented among prison and jailpopulations. \nPrisons and jails each year re-lease about 550,000 offenders withlatent tuberculosis. \nThe jail and prison settings areoften ill equipped to meet the healthneeds of inmates and many personsare released with only cursorytreatment or examinations.Thisrepresents a pool of illness that af-fects not only those with the dis-eases,but the employees of correc-tions facilities and the communitiesto which inmates return. \nOne major conclusion from thestudy is that developing means todiagnose and treat inmates will im-prove their lives, but may also im-prove the larger public's health andultimately save substantial amountsof money by earlier interventions. \nPage 2 \nWhen Congress authorized thestudy, the growing federal budgetsurplus held the promise that fundswould be available to help prisonsand jails improve health care forinmates. \nWith the federal budget now indeficit and more than half the statessuffering revenue shortfalls, thefindings of the commission arelikely to be shelved—despite theirominous conclusions.
