\nSummary \nOur study compared the effects of an angiotensin-converting enzyme inhibitor(captopril)versus a cal-cium antagonist (nifedipine)on proteinuria and renal function in patients with diabetic nephropathy.Arandomized follow-up study was designed.Type 2 diabetic patients, with established diabetic nephrop-athy(proteinuria>0.5g/24h),were treated with nifedipine(10 patients, group A)or captopril(10 pa-tients,group B)for 6 months.Arterial blood pressure, metabolic parameters, proteinuria and renalfunction were measured and compared.Mean perccntagc differcnces for glomcrular filtration ratc, renalplasma flow and filtration fraction between the two groups were calculated. No significant differenceswere observed in serum glucose, glycosylated hemoglobin(hemoglobin A₁c),Na+,K+or albumin ineither group or between groups.Blood pressure decreased significantly with both treatments and meanblood pressure was significantly lower in group A compared with group B at 6 months(Mann-WhitneyU-test,P=0.03).Proteinuria was similar in both groups at randomization, but after 3 and 6months oftreatment significant reductions were observed only in the group treated with captopril(P\u003C0.01).Asignificant decrease in filtration fraction was observed in group B with an increase in group A(Mann-Whitney U-test,P=0.03).Multiple regression analysis identified the therapeutic agent administered asan independent variable for decrease in proteinuria. It is concluded that antihypertensive treatment withcaptopril, but not with nifedipine, reduced proteinuria in patients with diabetic nephropathy, althougha better mean blood pressure was obtained with nifedipine. The difference in filtration fraction varia-tion between the two groups suggests an opposite pharmacological action on glomerular hemodynamicsand perhaps an explanation for the results on proteinuria of the different drugs. \nKey words:Type 2 diabetes mellitus;Diabetic nephropathy;Kidney function;Nifedipine;Captopril;Proteinuria \nsystolic blood pressure >140 mmHg),the valuesof systolic and diastolic blood pressure being con-sidered as the mean from two readings by theKorotkoff method (phases I and V),recorded atrest with a mercury sphygmomanometer.Smok-ing or consumption of alcohol or coffee was notallowed in the hour prior to reading of pressure. The mean blood pressure was calculated as di-astolic blood pressure plus a third of the pulsepressure.None of the patients had previouslybeen treated with angiotensin-converting enzymeinhibitors.Twenty-six patients over 57 years hadno contraindications to entry in the study.Twentyof these patients agreed to participate in the studyin accordance with the principles of the Declara-tion of Helsinki. \n# Introduction\n\nBetween 30 and 50%of patients with diabetesmellitus develop diabetic nephropathy during ev-olution of the disease[1].Mogensen in 1976[2],and other authors subsequently [3-5],haveshown that effective control of high blood pres-sure in diabetic renal disease reduces proteinuriaand the rate of impairment of renal function.Forthis reason, the control of hypertension has con-stituted one of the primary treatments for diabeticrenal disease [6].The selection of antihyperten-sive drug for the diabetic patient is under dispute[7-9],especially due to potential metabolic sideeffects [10,11].The angiotensin-converting en-zyme inhibitors exhibit fewer metabolic side ef-fects,and are thus being used to treat hyperten-sion in diabetic patients [12-14].However,thereare very few comparative studies in the literature[15,16]on the long-term effects of treatment ofhigh blood pressure in diabet","cbCaihpkMCQilW8q","https://ap.wps.com/l/cbCaihpkMCQilW8q","pdf",767468,"English","#
