However, the relationship between dietary patterns and colorectal cancer incidence is not conclusive. We aimed to assess the association between dietary patterns and colorectal cancer incidence in Brazilian states, using incidence estimates for the year 2020. This ecological population-based study uses the states of Brazil as analytical units. Food consumption data were obtained from the National Dietary Survey (NDS), a subsample of the Brazilian household budget survey (NDS-HBS) 2008–2009. Colorectal cancer incidence was obtained from the Brazilian National Cancer Institute according to sex. Dietary patterns were derived through factor analysis. Models with quasipoisson probability distributions were applied to associate dietary patterns with colorectal cancer incidence. Five dietary patterns were identified: Traditional Brazilian pattern-rice, beans, and red meat; Western pattern-soft drinks, sweetened beverages, snacks, fast foods, and candies; refined carbohydrate and fat pattern-bread, oils, and fats; healthy pattern – leafy and non-starchy vegetables, fruits, and eggs; and Dairy and beverage patterncoffee and teas, dairy products, and juices. After considering variables related to nutritional status and lifestyle, colorectal cancer in men was associated with Western Pattern (IRR 1.02; 95% CI 1.01–1.04); among women, associations were significant for Western Pattern (IRR 1.03; 95% CI 1.02–1.04), 3 (IRR 1.05; 95% CI 1.01–1.09), and Healthy Pattern (IRR 1.03; 95% CI 1.01–1.05) . Colorectal cancer was associated with certain Brazilian dietary patterns in both men and women. Thus, measures to promote adequate diet/nutrition in the Brazilian population should be strengthened and understood as strategies to prevent and control colorectal cancer.  \nKeywords Colorectal cancer, Dietary pattern, Brazilian household budget survey, Factor analysis, Generalized linear models  \nColorectal cancer (CRC) is among the five most incident cancer worldwide, and in Brazil. In 2022, there were an estimated 1.9 million new cases and an age-standardised incidence rate (ASR) of 19/100,000 inhabitants in the world, and 60,118 new cases (ASR: 19.8/100,000 inhabitants) in Brazil1. The disease will remain a public health concern in the coming decades; by 2030, there will be an 80% increase in incidence (2.2 million new cases) compared to 2008. Eight out often CRC cases are related to unhealthy lifestyles, including inadequate diet, high prevalence of physical inactivity, alcohol consumption, smoking, and overweight/obesity2–4.  \nThe Brazilian population has experienced a nutritional transition in recent decades, with reduced sources of dietary fibre such as rice and beans, alongside increased consumption of processed and ultra-processed foods5. Furthermore, increasing prevalence of overweight and decreased physical activity has been observed among Brazilian adults. More than half of Brazilian adults are overweight. At the same time, less than one-third engage in weekly sufficient levels of physical activity6. This epidemiological scenario corroborates projections of an increase in CRC cases in Brazil in the coming years7,8.  \nFew studies to date have evaluated the Brazilian diet and CRC burden. The existing studies have adopted mortality as the outcome9–11, and did not considered non-dietary risk factors for CRC such as overweight, smoking, alcohol consumption, and physical inactivity.
