VERIFICATION OF NURSE AIDE TRAINING OR NURSING SCHOOL TRAINING  \nComplete Section I of the Verification of Nurse Aide Training form (\\#08-4070b) . Submit the form to the school or facility where you completed your nurse aide training program. The training program will complete the form and return it to the Alaska Nurse Aide Registry.  \n4. VERIFICATION OF NURSE AIDE CERTIFICATION  \nComplete Section I of the Verification of Nurse Aide Certification form (\\#08-4070c) . Submit the form to the Nurse Aide Registry foryour initial and current state of certification. The Nurse Aide Registry will complete the form and return it to the Alaska Nurse Aide Registry.  \n5. FINGERPRINT & BACKGROUND CHECK  \nSubmit one original 8\" x 8\" fingerprint card (FD-258) . The card must be printed in black/pale blue ink, on cardstock and include the FBI privacy statement on the back.
