Her initial presentation mimicked postdural puncture headache (PDPH), but her condition evolved to include progressive neurological symptoms. Neuroimaging demonstrated extensive thrombosis of the cerebral venous sinuses with associated subdural hematomas. Prompt endovascular thrombectomy combined with therapeutic anticoagulation achieved full neurological recovery. This report highlights the diagnostic challenges of differentiating benign postpartum headaches from potentially life-threatening etiologies and underscores how pregnancy-related hypercoagulability and neuraxial procedures may interact to precipitate CVST. \n1 | Introduction \nHeadache during the postpartum period occurs in an estimated 10%–40% of women, with PDPH representing the most common cause after neuraxial anesthesia. However, clinicians must also consider cerebral venous sinus thrombosis (CVST), which is 10–20 times more frequent in pregnancy and the puerperium than in the general population. This heightened risk reflects theprothrombotic physiology of pregnancy, characterized by increased concentrations of clotting factors and reduced activity of endogenous anticoagulants. Although the association between neuraxial techniques and CVST is not fully elucidated, proposed mechanisms include mechanical trauma to epidural venous structures and altered cerebrospinal fluid dynamics, impairing venous drainage. Additional predisposing factors include dehydration, protracted labor, and instrumental delivery. This case illustrates the diagnostic dilemma when apparently typical PDPH conceals evolving CVST. The uniqueness of this case lies in the patient's concurrent presence of pregnancy-induced hypercoagulability and the potential mechanical injury and cerebrospinal \nfluid dynamic changes resulting from multiple epidural puncture attempts. This provides a dual-risk background for the occurrence of CVST, highlighting the necessity for high vigilance in similar clinical scenarios. We provide a detailed description of the clinical course, focusing on the decision points for neuroimaging and the importance of coordinated multidisciplinary care. \n2 | Case Presentation \n2.1 | Presenting Complaints and Disease History \nA 25-year-old primiparous woman at 38+3 weeks' gestation presented with 2 h of irregular lower abdominal pain. At 36+ weeks, her plasma D-dimer level was 4.26 mg/L, prompting initiation of prophylactic subcutaneous enoxaparin sodium 4000 AxaIU once daily. By 37 weeks, the plasma D-dimer level was 3.73 mg/L, while other coagulation parameters were within normal limits, and anticoagulation therapy was discontinued. During active labor, she requested neuraxial analgesia. \nThis is an open access article under the terms of the Creative Commons Attribution-NonCommercial-NoDerivs License, which permits use and distribution in any medium, provided the original work is properly cited, the use is non-commercial and no modifications or adaptations are made. \n© 2026 The Author(s).
