This obstructive pattern is typically non-reversible with bronchodilator therapy. Causes can range from toxic fumes, infections, and connective tissue disorders to complications from bone marrow or heart-lung transplants, and e-cigarette use. It is crucial to distinguish Bronchiolitis Obliterans from Cryptogenic Organizing Pneumonia, formerly known as Bronchiolitis Obliterans Organizing Pneumonia. Diagnosis involves pulmonary function testing showing an obstructive pattern with minimal response to bronchodilators, chest radiography which may be normal initially but can later show a reticular pattern with thickened airway walls, and HRCT of the chest revealing air trapping and patchy areas of decreased lung density. Trans-thoracic lung biopsies are considered the definitive diagnostic method, often showing mosaic attenuation, bronchiectasis, air trapping, and bronchial thickening. Differential diagnoses include Chronic Obstructive Pulmonary Disease (COPD), asthma, bronchiectasis, hypersensitivity pneumonitis, and pneumonia. Management focuses on slowing further worsening as the disease is irreversible, with treatments including corticosteroids, immunosuppressive medication, and lung transplantation. Routine vaccinations are recommended, and transplant recipients are at risk for re-developing the disease.
