Asthma is chronic inflammatory disorder of the airways characterized by recurrent episodes of variable airflow obstruction, often reversible. It is associated with bronchial inflammation involving T lymphocytes, mast cells, and eosinophils, leading to mucus plugging, oedema, smooth muscle hypertrophy, and epithelial damage. The condition is also marked by airway hyper-responsiveness to various stimuli. Pathophysiologically, asthma is considered a T-helper cell type 2 (Th2) driven disease, featuring thickened airway smooth muscle cells, subepithelial fibrosis, and aberrant immune regulation with increased cytokines like IL-4, IL-5, and IL-13. These cytokines contribute to eosinophilic inflammation, bronchial hyperreactivity, increased mucus production, and IgE synthesis. Other implicated cells include mast cells, neutrophils, macrophages, dendritic cells, and innate lymphoid cells. Etiologically, asthma's pathogenesis involves a complex interplay of factors. Classification can be based on predominant inflammatory cells, dividing asthma into T helper 2 high and T helper 2 low types. The T helper 2 high category further subdivides into atopic (allergic or extrinsic) asthma, intrinsic asthma, and exercise-induced asthma. Extrinsic asthma, common in children, is triggered by allergens like dust mites, is often seasonal, and may remit by teenage years. Intrinsic asthma, typically a late-onset condition, is linked to smoking and lacks atopy, with potential for irreversible obstruction.
